【摘要】：目的探討成年過(guò)敏性及非過(guò)敏性哮喘患者的臨床特征及意義。方法連續(xù)納入2014年10月至2016年6月于四川大學(xué)華西醫(yī)院哮喘專病門診初診未治療的成年哮喘患者,以抗原特異性IgE陽(yáng)性或常見過(guò)敏原皮膚點(diǎn)刺試驗(yàn)陽(yáng)性分為過(guò)敏性哮喘(AAS)組和非過(guò)敏性哮喘(NAAS)組,納入性別、發(fā)病年齡、哮喘控制測(cè)試(ACT)評(píng)分、肺功能、氣流受限類型、呼出氣一氧化氮分?jǐn)?shù)(FeNO)、體重指數(shù)、疾病嚴(yán)重程度等指標(biāo)進(jìn)行分析。結(jié)果共納入131例患者。其中,AAS組72例,NAAS組59例。NAAS組FeNO水平為(51.1±32.6)ppb,與AAS組(69.1±41.7)ppb相比,差異有統(tǒng)計(jì)學(xué)意義(t= 2.762,P=0.007);NAAS組合并鼻炎(17/59)與AAS組合并鼻炎(48/72)相比,差異有統(tǒng)計(jì)學(xué)意義(χ~2=19.396,P=0.000);NAAS組中無(wú)氣流受限(NAO)、可逆性氣流受限(RAO)、不可逆性氣流受限(IAO)分別為20例(33.9%)、22例(37.3%)、17例(28.8%),AAS組分別為37例(51.4%)、26例(36.1%)、9例(12.5%),兩組比較差異有統(tǒng)計(jì)學(xué)意義(Z= 2.461,P=0.014);NAAS組中輕度間歇/持續(xù)型、中度持續(xù)型、重度持續(xù)型分別為20例(33.9%)、18例(30.5%)、21例(35.6%),AAS組分別為37例(51.4%)、19例(26.4%)、16例(22.2%),兩組比較差異有統(tǒng)計(jì)學(xué)意義(Z= 2.115,P= 0.034);NAAS組發(fā)病年齡[(28.9±16.3)歲]、ACT評(píng)分[(15.6±4.1)分]、FEV1%pred[(67.64±22.81)%]、體重指數(shù)[(23.8±4.74)kg/m~2],與AAS組[分別為(25.5±15.0)歲,(16.7±4.2)分,(74.42±21.05)%,(22.5±4.1)kg/m~2]相比,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論與AAS相比,NAAS病情更重,合并鼻炎的較少,FeNO水平較低,發(fā)生IAO的比例更高。
[Abstract]:Objective to investigate the clinical features and significance of adult allergic and non-allergic asthma. Methods from October 2014 to June 2016, adult asthmatic patients who had not been treated at the clinic of asthma special disease, Huaxi Hospital, Sichuan University, were included in the study. Antigen-specific IgE positive or skin prick test positive of common allergens were divided into allergic asthma group and non-allergic asthma group, including gender, onset age, asthma control test score, pulmonary function, airflow limitation type. The exhaled nitric oxide score (FES), body mass index (BMI) and severity of disease were analyzed. Results 131 patients were included. The FeNO level in 72 cases of AAS group was 51.1 鹵32.6ppb, which was significantly higher than that of AAS group (69.1 鹵41.7)ppb). There was a significant difference between AAS group and AAS group (1759%) and AAS group combined with rhinitis group (48P / 72).
