沙丁胺醇對COPD兔支氣管插管單肺通氣中LTB4濃度的影響

【摘要】:目的觀察沙丁胺醇對煙熏致慢性阻塞性肺疾病(COPD)實驗兔模型單肺通氣后血清和肺泡灌洗液中白三烯B4(LTB4)濃度的影響。方法健康新西蘭大白兔18只分為正常組、COPD組、COPD+沙丁胺醇組,每組6只。除正常組以外的其他兩組均采用實驗組自行創(chuàng)造的方法構建了單純煙熏致COPD實驗兔模型。分別在給實驗兔行右主支氣管插管前(T0),插管后即刻(T1),插管后行單肺通氣30min(T2),插管后行單肺通氣60min(T3)采血測LTB4,行右主支氣管插管單肺通氣60min后處死實驗兔,解剖后取少量雙肺上葉作組織病理學觀察,取適量支氣管肺泡灌洗液測LTB4的濃度。結果 COPD組中實驗兔支氣管肺泡灌洗液中LTB4的濃度比正常組明顯升高(P0.05),COPD+沙丁胺醇組中LTB4的濃度比COPD組明顯降低(P0.05)。肺組織形態(tài)學檢查顯示,COPD組觀察到炎性細胞侵入,肺組織有明顯的炎癥性改變;COPD+沙丁胺醇組可見炎性細胞明顯減少,肺組織炎癥性改變有一定改善。結論支氣管插管單肺通氣中吸入沙丁胺醇能抑制LTB4的濃度。
[Abstract]:Objective to observe the effect of salbutamol on the concentration of leukotriene B _ 4 and LTB _ 4 in serum and alveolar lavage fluid after single lung ventilation in fumigated chronic obstructive pulmonary disease (COPD) rabbits. Methods 18 healthy New Zealand rabbits were divided into normal group and COPD group with 6 in each group. In addition to the normal group, all the other two groups established the COPD experimental rabbit model by the method of self-created by the experimental group. The blood samples were collected before right main bronchus intubation, immediately after intubation, 30 minutes after intubation and 60 minutes after intubation. LTB4 was measured. The rabbits were killed after right main bronchus intubation with single lung ventilation (60min). After dissection, a small amount of upper lobes of lung were taken for histopathological observation, and appropriate amount of bronchoalveolar lavage fluid (BALF) was taken to measure the concentration of LTB4. Results the concentration of LTB4 in bronchoalveolar lavage fluid in COPD group was significantly higher than that in normal group. The concentration of LTB4 in COPD group was significantly lower than that in COPD group. Pulmonary histomorphologic examination showed that inflammatory cell invasion was observed in COPD group, and there were obvious inflammatory changes in lung tissue. In COPD salbutamol group, inflammatory cells were significantly decreased, and inflammatory changes in lung tissue were improved to some extent. Conclusion inhalation of salbutamol can inhibit the concentration of LTB4 during single lung ventilation.
