【摘要】：目的采用中醫(yī)外敷三伏貼聯(lián)合補(bǔ)肺活血膠囊內(nèi)服治療慢性阻塞性肺疾病(COPD)穩(wěn)定期,通過(guò)觀察其治療后總療效、中醫(yī)癥候積分等的改變,評(píng)價(jià)三伏貼聯(lián)合補(bǔ)肺活血膠囊治療慢性阻塞性肺疾病的臨床療效,以期在探討其內(nèi)在作用理論的同時(shí),為中醫(yī)藥防治COPD提供新的思路和方向。方法1.選取符合COPD穩(wěn)定期診斷標(biāo)準(zhǔn)患者70例,肺功能分級(jí)為I~III級(jí),病情分期為穩(wěn)定期。患者均來(lái)自于2015年-2016年2年期間在湖北省中醫(yī)院呼吸科門診或住院部治療的病例,按就診的先后順序?qū)⒒颊叻譃?組,治療組及對(duì)照組各35例。兩組患者在年齡、性別、病程、病情分級(jí)、治療前中醫(yī)癥狀積分、生活質(zhì)量評(píng)分、6分鐘步行距離等資料經(jīng)統(tǒng)計(jì)學(xué)分析無(wú)明顯差異,具有可比性(P0.05)。2.對(duì)照組予以噻托溴銨粉霧劑18ug(商品名:思力華勃林格殷格翰公司)治療,每天1粒。治療組予以中藥三伏貼聯(lián)合補(bǔ)肺活血膠囊治療。三伏貼外敷:藥物由白芥子、延胡索、細(xì)辛、甘遂、肉桂等按一定的比例研磨成細(xì)粉,再以生姜汁攪拌調(diào)勻,制成膏狀;時(shí)間為2015年至2016年初伏、中伏、末伏的第一天進(jìn)行敷貼;取穴為雙側(cè)大杼、肺俞、厥陰俞、腎俞,若喘氣明顯者加定喘,咳痰量多者加豐隆;每次敷貼時(shí)間為2-4小時(shí),若敷貼后皮膚刺癢難忍,灼熱疼痛則應(yīng)立即取下,對(duì)敷貼無(wú)特殊不適者可適當(dāng)延長(zhǎng)時(shí)間,但不宜超過(guò)6小時(shí)。補(bǔ)肺活血膠囊(由黃芪、赤芍、補(bǔ)骨脂組成,廣東遠(yuǎn)大藥業(yè)有限公司)內(nèi)服:每天3次,每次4粒。2組均以2年為1個(gè)療程,1療程結(jié)束后評(píng)定其臨床療效。兩組患者在治療期間需戒煙酒,避風(fēng)寒,禁食辛辣刺激食物,以免影響觀察效果。3.療效標(biāo)準(zhǔn):顯效:主要臨床癥狀改善,療效指數(shù)≥70%;有效:主要臨床癥狀改善,療效指數(shù)≥30%,而70%;無(wú)效:臨床癥狀無(wú)改善,療效指數(shù)30%。4.統(tǒng)計(jì)學(xué)分析方法:數(shù)據(jù)采用SPSS19.0統(tǒng)計(jì)學(xué)軟件進(jìn)行分析。計(jì)數(shù)資料使用X2檢驗(yàn),計(jì)量資料使用t檢驗(yàn)。結(jié)果1.本次研究中治療組顯效8例,有效19例,無(wú)效8例,總有效率為77.14%;對(duì)照組中顯效8例,有效17例,無(wú)效10例,總有效率為71.43%。兩組總有效率對(duì)比無(wú)明顯差異(P0.05),療效相當(dāng)。2.兩組患者治療后中醫(yī)癥候積分和治療前相比均有差異(P0.05),且治療組優(yōu)于對(duì)照組(P0.05)。3.兩組患者治療后生活質(zhì)量積分和治療前相比均有差異(P0.05),且治療組生活質(zhì)量高于對(duì)照組(P0.05)。4.兩組患者治療后一年內(nèi)感冒次數(shù)均較前減少(P0.05),治療組年感冒次數(shù)少于對(duì)照組(P0.05)。5.兩組患者治療后6分鐘步行距離均較前有所增加(P0.05),治療組步行距離增加明顯高于對(duì)照組(P0.05)。結(jié)論此次研究表明外敷三伏貼聯(lián)合內(nèi)服補(bǔ)肺活血膠囊治療慢性阻塞性肺疾病穩(wěn)定期有其優(yōu)勢(shì),可明顯改善患者臨床癥狀,減少一年內(nèi)感冒次數(shù),提高運(yùn)動(dòng)耐力及生活質(zhì)量,與西藥比較,通過(guò)內(nèi)外同治調(diào)節(jié)機(jī)體免疫功能,調(diào)和經(jīng)絡(luò)氣血,增強(qiáng)人體正氣,提高抵御外邪的能力,治療效果顯著,值得在臨床上進(jìn)一步推廣使用。
[Abstract]:Objective to observe the changes of the total curative effect and TCM symptom score after the treatment of chronic obstructive pulmonary disease (COPD) with external application of Sanfu plaster and internal administration of Bufei Huoxue capsule in chronic obstructive pulmonary disease (COPD). To evaluate the clinical efficacy of Sanfu tie combined with Bufei Huoxue capsule in the treatment of chronic obstructive pulmonary disease (COPD) in order to provide new ideas and directions for the prevention and treatment of COPD by traditional Chinese medicine (TCM). Method 1. Seventy patients with COPD stable stage were selected. The pulmonary function grade was I~III grade and the disease stage was stable stage. The patients were all from the outpatient department or inpatient department of respiratory department of Hubei traditional Chinese Medicine Hospital from 2015 to 2016. The patients were divided into two groups according to the order of treatment, 35 cases in the treatment group and 35 cases in the control group. There was no significant difference between the two groups in age, sex, course of disease, grade of illness, score of TCM symptom before treatment, quality of life score and 6-minute walking distance. There was no significant difference between the two groups by statistical analysis, and there was no significant difference in comparison between the two groups. The control group was treated with thiotropium bromide powder (18ugg). The treatment group was treated with Sanfu paste and Bufei Huoxue capsule. External application of three volts: the medicine is ground into fine powder by white mustard, corydalis, asarum, kansui and cinnamon in a certain proportion, and then mixed with ginger juice to make a paste. The time is from 2015 to early 2016. At the end of the first day, the points were applied on both sides of the loom, Feshu, Jueyin Yu, Shenshu, if the breath was obvious, and the expectoration was much more. The time of each application was 2-4 hours, and if the skin prickled after the application, it was hard to bear itching. Burn pain should be removed immediately, the application of no special discomfort can be extended appropriately, but not more than 6 hours. Bufei Huoxue capsule (composed of Radix Astragali, Radix Paeoniae Paeoniae Alba, psoralen, Guangdong Broad Pharmaceutical Co., Ltd.) was taken orally: 3 times a day, 4 tablets per day, 2 years as a course of treatment and 1 course of treatment were used to evaluate its clinical effect. Two groups of patients in the treatment period should quit smoking and alcohol, avoid cold, fasting spicy food, so as not to affect the observation effect. 3.
