VEGF在良惡性胸腔積液中表達(dá)差異的研究

【摘要】:目的:通過檢測(cè)結(jié)核性胸膜炎及惡性胸腔膜炎患者血清和胸水中VEGF含量, 探討VEGF在結(jié)核性胸膜炎及惡性胸膜炎患者血清、胸水中表達(dá)的差異,并分析VEGF在不同原因胸膜炎的胸水和血清中是否存在相關(guān)性。 方法:以經(jīng)可彎曲電子胸腔鏡下取胸膜活檢經(jīng)病理確診的結(jié)核性胸膜炎和惡性胸膜炎患者各30例為研究對(duì)象,對(duì)所有患者在進(jìn)行可彎曲電子胸腔鏡檢查術(shù)中留取胸水標(biāo)本10ml,并在手術(shù)當(dāng)天取患者靜脈血5m1標(biāo)本,采用雙抗體夾心酶聯(lián)免疫吸附實(shí)驗(yàn)(ELISA)檢測(cè)患者胸水及血清中VEGF水平。兩組患者血清和胸水VEGF差異比較采用t檢驗(yàn),兩組患者胸水與血清VEGF相關(guān)分析采用直線相關(guān)及逐步回歸分析。 結(jié)果:結(jié)核性胸膜炎患者血清和胸水VEGF檢測(cè)值分別為45.33±18.33pg/mL,62.73±24.65pg/mL惡性胸膜炎血清和胸水VEGF檢測(cè)值分別為66.00±29.83pg/mL,95.54±42.11pg/mL。將結(jié)核性胸膜炎組與惡性胸膜炎組的血清、胸水中的VEGF含量進(jìn)行比較,胸水中VEGF含量均高于血清中的含量(t值分別為2.61,3.315,均P0.05),差異有統(tǒng)計(jì)學(xué)意義。將惡性胸膜炎和結(jié)核性胸膜炎患者的血清、胸水中VEGF含量進(jìn)行比較,惡性胸膜炎患者的血清及胸水中VEGF含量均高于結(jié)核性胸膜炎患者(t值分別為3.9,5.2,均P0.05),差異有統(tǒng)計(jì)學(xué)意義。對(duì)結(jié)核性胸膜炎患者與惡性胸膜炎患者血清和胸水VEGF檢測(cè)值進(jìn)行相關(guān)性分析(R值分別為0.53,0.38,均P0.05),VEGF在結(jié)核性胸膜炎和惡性胸膜炎的血清和胸水中均呈正相關(guān)性。 結(jié)論:VEGF在結(jié)核性胸膜炎及惡性胸膜炎患者血清、胸水中的表達(dá)有差異,惡性高于結(jié)核性;兩組患者胸水中VEGF含量均高于血清,胸水VEGF含量隨著血清VEGF含量增高而增高。檢測(cè)胸腔積液患者血清和胸水中VEGF含量對(duì)結(jié)核性胸膜炎和惡性胸膜炎的診斷和鑒別診斷有一定的價(jià)值。
[Abstract]:Objective: to detect the content of VEGF in serum and pleural effusion in patients with tuberculous pleurisy and malignant pleurisy.To investigate the difference of VEGF expression in serum and pleural effusion of patients with tuberculous pleurisy and malignant pleurisy, and to analyze the correlation between VEGF in pleural effusion and serum of different causes pleurisy.Methods: 30 cases of tuberculous pleurisy and 30 cases of malignant pleurisy confirmed pathologically by flexible electronic thoracoscopy were studied.Samples of pleural effusion were collected from all patients during the operation of bendable electron thoracoscopy, and venous blood 5m1 samples were collected on the day of operation. The VEGF levels in pleural effusion and serum were detected by double antibody sandwich enzyme-linked immunosorbent assay (Elisa).The difference of VEGF between serum and pleural effusion was analyzed by t test. Linear correlation and stepwise regression analysis were used to analyze the correlation between pleural effusion and serum VEGF in the two groups.Results: the detection values of VEGF in serum and pleural effusion of tuberculous pleurisy patients were 45.33 鹵18.33pg / mL0.73 鹵24.65pg/mL and 66.00 鹵29.83pg / mL, respectively. The detection values of VEGF were 95.54 鹵42.11pgmLrespectively.The contents of VEGF in serum and pleural effusion of tuberculous pleurisy group and malignant pleurisy group were compared. The VEGF content in pleural effusion was higher than that in serum was 2.61 鹵3.315, respectively (P 0.05). The difference was statistically significant.The contents of VEGF in serum and pleurisy of patients with malignant pleurisy and tuberculous pleurisy were compared. The VEGF content in serum and pleurisy of malignant pleurisy patients was higher than that in tuberculous pleurisy patients (P 0.05).The correlation analysis of serum and pleural effusion VEGF detection between patients with tuberculous pleurisy and malignant pleurisy was carried out. The R value was 0. 53 鹵0. 38, respectively. There was a positive correlation between serum and pleural effusion of tuberculous pleurisy and malignant pleurisy.Conclusion the expression of VEGF in pleural effusion of tuberculous pleurisy and malignant pleurisy is higher than that of tuberculous pleurisy, and the content of VEGF in pleural effusion is higher than that in serum, and the content of VEGF in pleural effusion increases with the increase of serum VEGF content.Detection of VEGF in serum and pleural effusion is valuable in the diagnosis and differential diagnosis of tuberculous pleurisy and malignant pleurisy.
