【摘要】：目的探討穩(wěn)定期慢性阻塞性肺疾病(COPD)患者認知功能的影響因素。方法選取2013年1月-2014年11月期間該院收治的128例穩(wěn)定期COPD患者為病例組,并選擇同期來該院體檢的75名健康成人為對照組。比較兩組及病例組中各亞組的蒙特利爾認知評估量表(Mo CA)評分,通過簡單線性相關和多重線性回歸分析患者的Mo CA評分與各因素的關系。結果病例組的Mo CA評分顯著低于對照組(P0.05)。年齡、吸煙指數(shù)、受教育時間、COPD嚴重程度、病程、Pa O2、Pa CO2、SO2、SAS及SDS評分的各亞組的Mo CA評分比較,差異有統(tǒng)計學意義(P0.05)。Mo CA評分與Pa O2、SO2成正相關,與年齡、COPD嚴重程度、SDS評分負相關(P0.05)。結論穩(wěn)定期COPD患者存在不同程度的認知功能障礙,高齡、肺功能顯著降低、低氧血癥、抑郁表現(xiàn)是患者發(fā)生認知功能障礙的高危因素,應進行早期診治。
[Abstract]:Objective to investigate the influencing factors of cognitive function in patients with stable chronic obstructive pulmonary disease (cod). Methods 128 patients with stable COPD from January 2013 to November 2014 were selected as the case group. 75 healthy adults in the same period were selected as control group. The scores of Montreal Cognitive Assessment scale (MCAS) were compared between the two groups and each subgroup of the case group. By simple linear correlation and multiple linear regression analysis, the relationship between the scores of Mo CA and various factors was analyzed. Results the scores of Mo CA in the case group were significantly lower than those in the control group (P 0.05). Age, smoking index, time of education were significantly lower than those of the control group. There was a positive correlation between the scores of Mo CA and PAO _ 2 O _ 2 so _ 2 in different subgroups of PAO _ 2 / CO _ 2O _ 2SO _ 2 and SDS scores. There was a positive correlation between the scores of Mo CA and so _ 2 of Pao _ 2O _ 2, the difference was significant (P < 0.05). Conclusion the patients with COPD at stable stage have different degree of cognitive dysfunction, the elderly, the decrease of pulmonary function, hypoxemia and depression are the high risk factors of cognitive dysfunction. Early diagnosis and treatment should be carried out.
