【摘要】：目的：研究經(jīng)纖維支氣管鏡針吸微創(chuàng)介入方法對老年上縱隔囊腫診治的臨床應(yīng)用。 方法：回顧性分析2003年06月至2012年08月吉林大學(xué)第二醫(yī)院呼吸內(nèi)科收治的8例縱隔囊腫患者的臨床資料，均應(yīng)用日本OLYMPUSBF1T260型電子纖維支氣管鏡和日本OLYMPUS BFN1C細(xì)胞學(xué)穿刺針行針吸活檢和穿刺排液治療，并進(jìn)行隨訪觀察療效。結(jié)合文獻(xiàn)資料，進(jìn)行相關(guān)分析。 結(jié)果：8例縱隔囊腫患者年齡60-78歲，其中男6例，女2例。有癥狀3例，無癥狀5例。胸部影像學(xué)顯示均為上縱隔囊腫。經(jīng)細(xì)胞學(xué)病理檢查，均排除惡變可能，對其進(jìn)行細(xì)針穿刺排液治療后，隨訪6個(gè)月均未復(fù)發(fā)。 結(jié)論：對老年上縱隔囊腫患者，經(jīng)纖維支氣管鏡針吸微創(chuàng)介入療法的可行性、安全性、有效性均良好，可作為首選治療方案。在排除惡性病變的同時(shí)，可對囊腫進(jìn)行穿刺排液治療，在部分患者中療效良好。對于復(fù)發(fā)患者，，推薦硬化劑的使用。
[Abstract]:Objective: to study the clinical application of minimally invasive interventional method with fiberoptic bronchoscope in the diagnosis and treatment of superior mediastinal cyst in the elderly. Methods: the clinical data of 8 patients with mediastinal cysts treated in Department of Respiratory, second Hospital of Jilin University from June 2003 to August 2012 were retrospectively analyzed. All patients were treated with OLYMPUSBF1T260 electronic fiberoptic bronchoscope and OLYMPUS BFN1C cytological puncture needle, and followed up to observe the curative effect. Results the age of 8 cases of mediastinal cyst was 60-78 years old, including 6 males, 2 females, 3 symptomatic cases and 5 asymptomatic cases. The chest imaging showed that all of them were upper mediastinal cysts. After fine needle puncture and drainage treatment, there was no recurrence after 6 months follow-up. Conclusion: in elderly patients with superior mediastinal cysts, minimally invasive interventional therapy via fiberoptic bronchoscopy is feasible, safe and effective, and can be used as the first choice of treatment. The cysts can be treated by puncture and drainage, which has a good effect in some patients. Sclerosing agent is recommended for recurrent patients.
