【摘要】：目的探討血清糖類抗原125(CA125)水平與慢性阻塞性肺疾病(COPD)患者肺動(dòng)脈壓力的關(guān)系,研究CA125是否在COPD合并肺動(dòng)脈高壓(pulmonary hypertension,PH)中具有診斷價(jià)值。方法收錄穩(wěn)定期COPD患者80例,其中合并PH者29例。對(duì)比有無PH患者血清CA125水平,分析CA125與心臟彩超肺動(dòng)脈收縮壓(systolic pulmonary artery pressure,s PAP)測(cè)定值的相關(guān)性,對(duì)CA125預(yù)測(cè)COPD患者是否合并PH繪制ROC曲線。結(jié)果合并PH患者的血清CA125水平明顯高于無PH患者(Z=-3.40,P0.05),合并PH患者平均s PAP為(59.72±12.61)mm Hg,與其血清CA125水平無相關(guān)性(P0.05)。評(píng)估CA125與心臟彩超在PH診斷中的一致性,Kappa值為0.307,對(duì)CA125預(yù)測(cè)COPD是否合并PH繪制ROC曲線,得出曲線下面積(AUC)為0.723,95%可信區(qū)間為0.600~0.875,根據(jù)ROC曲線得出CA125為59.66 U/m L時(shí),Youden指數(shù)最大為0.424,此時(shí)預(yù)測(cè)COPD患者合并PH的敏感度為48.3%,特異度為94.1%。結(jié)論 CA125測(cè)定簡(jiǎn)單易行,其水平可協(xié)助判斷COPD患者是否合并PH。
[Abstract]:Objective to investigate the relationship between serum carbohydrate antigen 125 (CA125) and pulmonary artery pressure in patients with chronic obstructive pulmonary disease (COPD), and to investigate the diagnostic value of CA125 in patients with COPD complicated with pulmonary hypertension. Methods 80 patients with stable COPD were included in this study. Among them, 29 cases were complicated with PH. To compare the serum CA125 level of patients with PH or not, to analyze the correlation between CA125 and systolic pulmonary artery pressure PAPs. Results the serum CA125 level in patients with CA125 was significantly higher than that in patients without PH, and the average s PAP in patients with PH was 59.72 鹵12.61 mm Hg.There was no correlation between serum CA125 level and CA125 level in patients with PH. The consistency of color Doppler ultrasound in the diagnosis of PH was 0.307, and the value of Kappa was 0.307. Whether the COPD was combined to draw the ROC curve was predicted by CA125. According to the ROC curve, the maximum value of Youden index was 0.424 when CA125 was 59.66 UmL, and the sensitivity of predicting PH in COPD patients was 48.3% and the specificity was 94.1.Conclusion CA125 is simple and feasible. Its level can help to judge whether COPD patients are associated with COPD.
