【摘要】：目的:觀察重組人血管內(nèi)皮抑制素結(jié)合CT引導下經(jīng)皮穿刺微波消融術(shù)治療晚期非小細胞肺癌(NSCLC)合并慢性阻塞性肺疾病(COPD)的臨床療效及安全性。方法:選擇2014年2月-2016年2月我院收治的80例晚期NSCLC合并COPD患者,按隨機數(shù)字表法分為對照組和觀察組,各40例。對照組患者給予CT引導下經(jīng)皮穿刺微波消融術(shù)治療;觀察組患者在對照組基礎(chǔ)上加用重組人血管內(nèi)皮抑制素注射液7.5 mg/m2靜脈滴注,4 h滴完,d1~14,連續(xù)治療14 d后休息7 d再進行下一周期治療。每個治療周期為21 d,共治療4個周期。比較兩組患者的生存時間、臨床療效、治療前后的卡氏(KPS)評分和肺功能指標,以及不良反應發(fā)生情況。結(jié)果:觀察組患者的中位生存時間(19.8個月)明顯長于對照組(15.2個月),總有效率(72.5%)明顯高于對照組(55.0%),差異均有統(tǒng)計學意義(P0.05)。治療前,兩組患者的KPS評分、1 s用力呼氣容量(FEV1)、FEV1/用力肺活量(FVC)和肺一氧化碳彌散量(DLCO)比較,差異均無統(tǒng)計學意義(P0.05);治療后,兩組患者的KPS評分和上述肺功能指標水平均較治療前明顯升高,且觀察組均明顯高于對照組,差異均有統(tǒng)計學意義(P0.05)。兩組患者均未見Ⅳ級不良反應發(fā)生,各級別不良反應發(fā)生例數(shù)比較,差異均無統(tǒng)計學意義(P0.05)。結(jié)論:重組人血管內(nèi)皮抑制素結(jié)合CT引導下經(jīng)皮穿刺微波消融術(shù)治療晚期NSCLC合并COPD的臨床療效較好、不良反應較輕,能明顯改善患者的肺功能和生存質(zhì)量。
[Abstract]:Objective: to observe the clinical efficacy and safety of recombinant human vascular endotheliostatin combined with CT guided percutaneous microwave ablation in the treatment of advanced non-small cell lung cancer (NSCLC) with chronic obstructive pulmonary disease (COPD). From June to February 2016, 80 patients with advanced NSCLC complicated with COPD were treated in our hospital.
