【摘要】：目的分析1~4級肺動脈(PA)管徑與肺功能及CT肺容積參數(shù)的相關(guān)性。方法通過曲面重組方法測量48例慢性阻塞性肺疾病(COPD)患者肺動脈干(A1)、左/右肺動脈(LA2/RA2)、右肺上葉肺動脈(RUA3)、左/右肺下葉肺動脈(LLA3/RLA3)、右肺上葉尖段肺動脈(RUA4)、左肺上葉前段肺動脈(LUA4)、右肺中葉外段肺動脈(RMA4)、左/右肺下葉背段肺動脈(LLA4/RLA4)的管徑,評價其與肺功能指標(biāo)FEV1、FEV1%、FVC、FEV1/FVC、MEF25、MEF25%及CT肺容積參數(shù)[全肺容積(TLV)、肺氣腫容積(TEV)、肺氣腫指數(shù)(EI)]的關(guān)系。結(jié)果第1級PA管徑慢性阻塞性肺疾病全球倡議(GOLD)僅Ⅰ與Ⅱ級、Ⅱ與Ⅲ級存在差異(P=0.009,P=0.002)。第2級PA管徑GOLDⅠ與Ⅱ級、Ⅱ與Ⅲ級、Ⅱ與Ⅳ級存在差異(P=0.003;P=0.034,P=0.043;P=0.041)。第3級PA管徑GOLDⅠ與Ⅲ、Ⅰ與Ⅳ級存在差異(P=0.004,P=0.024;P=0.009,P=0.003)。第4級PA管徑GOLDⅠ與Ⅱ、Ⅰ與Ⅲ、Ⅰ與Ⅳ、Ⅱ與Ⅳ及Ⅲ與Ⅳ級存在差異(P=0.015;P=0.019,P=0.000;P=0.000,P=0.000,P=0.015;P=0.006,P=0.006,P=0.006,P=0.018;P=0.027)。第1~2級PA管徑與肺功能指標(biāo)無相關(guān)性。第3級PA管徑與肺功能指標(biāo)呈弱負(fù)相關(guān)性(r=-0.289~-0.486)。第4級PA管徑與肺功能指標(biāo)呈中等正相關(guān)性(r=0.378~0.716)。第1~2級PA管徑與CT肺容積參數(shù)無相關(guān)性,第3級部分PA管徑與CT肺容積參數(shù)呈中等正相關(guān)性(r=0.415~0.551),第4級PA除RUA4、LUA4外,其余4級PA管徑均與所有CT肺容積參數(shù)呈弱至中等負(fù)相關(guān)性(r=-0.366~-0.717)。結(jié)論隨著COPD患者PA管徑的變細(xì),不同GOLD級別間的差異也越顯著。第3級PA管徑隨著肺功能指標(biāo)的減小和CT肺容積參數(shù)的增高而增寬,而第4級PA管徑則變窄。第4級PA管徑與肺功能指標(biāo)及EI的相關(guān)性較第3級PA大。FEV1%、FEV1/FVC及TEV、EI與第3~4級PA的相關(guān)性較高。
[Abstract]:Objective to analyze the correlation between PAA diameter and pulmonary function and CT pulmonary volume parameters. Methods the pulmonary trunk A1D, left / right pulmonary artery (LA2 / RA2) and right upper lobe lung were measured by curved recombination method in 48 patients with chronic obstructive pulmonary disease (COPD). The diameter of the left / right lower lobe pulmonary artery (LLA3 / RLA3), the right superior lobe pulmonary artery (RUA4), the left superior lobe anterior pulmonary artery (LUA4), the right lateral middle lobe pulmonary artery (RMA4), the left / right inferior lobe pulmonary artery LLA4 / RLA4).
