【摘要】：目的探討塵肺病住院患者發(fā)生醫(yī)院感染的狀況并分析其危險因素。方法對本科2006年1月至2013年12月確診為塵肺病的138例住院患者病例資料進(jìn)行回顧性分析。結(jié)果 138例塵肺病患者中39例發(fā)生了醫(yī)院感染,醫(yī)院感染發(fā)生率為28.26%;檢出病原菌16株,檢出率為37.21%。單因素分析結(jié)果顯示,感染組塵肺分期、住院時間、吸煙、血清白蛋白、抗生素使用時間與非感染組比較,差異有統(tǒng)計學(xué)意義(P0.05,P0.01)。多因素分析結(jié)果顯示,塵肺分期(OR=0.019,P=0.004,95%CI:0.001~0.278)、住院時間(OR=1.085,P=0.001,95%CI:1.035~1.137)、抗生素使用時間(OR=1.112,P=0.024,95%CI:1.014~1.220)是其主要危險因素,而血清白蛋白(OR=0.841,P=0.012,95%CI:0.734~0.963)是其保護(hù)因素。結(jié)論塵肺病住院患者醫(yī)院感染發(fā)生率較高,塵肺分期越嚴(yán)重、住院時間越長、使用抗生素時間越長、血清白蛋白越低,醫(yī)院感染風(fēng)險越大。
[Abstract]:Objective to investigate the status of nosocomial infection in patients with pneumoconiosis and to analyze its risk factors. Methods the data of 138 inpatients with pneumoconiosis from January 2006 to December 2013 were retrospectively analyzed. Of 138 patients with pneumoconiosis, 39 had nosocomial infections. The incidence of nosocomial infection was 28.26, 16 strains of pathogenic bacteria were detected, the detection rate was 37.21. The results of univariate analysis showed that the pneumoconiosis stage, hospitalization time, smoking, serum albumin, antibiotic use time of infection group were compared with those of non-infection group. The results of multivariate analysis showed that the main risk factors were the stage of pneumoconiosis (OR 0.019), the percent of CI: 0.001 / 95, the length of stay, the duration of hospitalization, the duration of antibiotic use, and the duration of antibiotic use, OR1.112P0.02495CIW 1.014 / 1.220). Conclusion the incidence of nosocomial infection in patients with pneumoconiosis is higher, the stage of pneumoconiosis is more serious, the longer the length of hospitalization, the longer the use of antibiotics, the lower the serum albumin, the greater the risk of nosocomial infection.
