【摘要】：目的探討平喘固本湯加減聯(lián)合家庭氧療治療穩(wěn)定期慢性阻塞性肺疾病(COPD)的療效及其對(duì)患者認(rèn)知功能障礙預(yù)防的價(jià)值。方法80例穩(wěn)定期COPD患者隨機(jī)分為對(duì)照組和研究組各40例。對(duì)照組給予祛痰、平喘等常規(guī)治療,研究組在對(duì)照組的基礎(chǔ)上給予固本止咳平喘顆粒聯(lián)合家庭氧療治療。測(cè)定兩組治療前后肺功能指標(biāo)和動(dòng)脈血?dú)夥治龅淖兓?比較兩組治療前后呼吸困難(m MRC)、生活質(zhì)量(SGRQ)以及認(rèn)知功能(MMSE)評(píng)分,比較兩組認(rèn)知功能障礙的發(fā)生率。結(jié)果研究組治療后m MRC和SGRQ評(píng)分,顯著低于對(duì)照組,MMSE評(píng)分顯著高于對(duì)照組(P0.05);研究組治療后用力肺活量(FVC)、第1秒用力呼氣量(FEV1)和最大呼氣峰流速值(PEF)均顯著高于對(duì)照組(P0.05);研究組治療后動(dòng)脈血氧分壓(Pa O2)顯著高于對(duì)照組,動(dòng)脈二氧化碳分壓(Pa CO2)顯著低于對(duì)照組(P0.05);研究組治療后認(rèn)知功能障礙發(fā)生率顯著低于對(duì)照組(P0.05)。結(jié)論平喘固本湯加減聯(lián)合家庭氧療治療可在改善穩(wěn)定期COPD患者肺功能的同時(shí)大大降低患者認(rèn)知功能障礙的發(fā)生風(fēng)險(xiǎn),改善患者的生活質(zhì)量,值得臨床應(yīng)用。
[Abstract]:Objective to explore the treatment of stable chronic obstructive pulmonary disease (COPD) by combined therapy of Pingchuan Guben decoction and family oxygen therapy. Methods 80 patients with stable COPD were randomly divided into control group (n = 40) and study group (n = 40). On the basis of the control group, the study group was treated with Guben Zhike Pingchuan granule combined with family oxygen therapy. The changes of pulmonary function index and arterial blood gas analysis were measured before and after treatment in both groups. The scores of dyspnea, quality of life (QOL) and cognitive function (MMSE) were compared between the two groups before and after treatment. Results the scores of m MRC and SGRQ in the study group were significantly lower than those in the control group (P 0.05). After treatment, forced vital capacity (FV), forced expiratory volume (FEV1) and peak expiratory peak flow (PEFV) in the study group were significantly higher than those in the control group (P 0.05). After treatment, the arterial partial pressure of oxygen (Pao _ 2) in the study group was significantly higher than that in the control group, and the arterial partial pressure of CO _ (2) was significantly lower than that in the control group (P 0.05). The incidence of cognitive dysfunction in the study group was significantly lower than that in the control group (P 0.05). Conclusion the combination of Pingchuan Guben decoction and family oxygen therapy can improve the pulmonary function of patients with stable COPD and reduce the risk of cognitive dysfunction. It is worthy of clinical application to improve the quality of life of patients.
