【摘要】：經(jīng)支氣管鏡肺減容術(shù)（BLVR）是國(guó)際上治療重度慢性阻塞性肺疾�。–OPD）合并肺氣腫患者的熱點(diǎn)技術(shù)，它可經(jīng)多種方法實(shí)現(xiàn)，如支氣管內(nèi)活瓣置入（EBV）、氣道旁路支架、熱蒸汽消融、生物肺減容、支氣管內(nèi)線圈置入、支氣管內(nèi)封堵等技術(shù)。目前國(guó)內(nèi)外臨床研究最多的是EBV，研究顯示具備高異質(zhì)性肺氣腫，葉間裂完整和靶肺葉無(wú)旁路通氣三要素的患者EBV療效最好。新近出現(xiàn)的高分辨率CT（HRCT）及Chartis評(píng)估系統(tǒng)可以系統(tǒng)解決上述三要素問題，提高了EBV成功率及療效，極具開發(fā)應(yīng)用前景。文中綜述了HRCT與Chartis評(píng)估系統(tǒng)在EBV治療重度肺氣腫中機(jī)制及應(yīng)用進(jìn)展。
[Abstract]:Transbronchial pulmonary volume reduction (BLVR) is a hot technique in the treatment of severe chronic obstructive pulmonary disease (COPD) with emphysema. It can be realized by many methods. Such as endobronchial valve implantation, airway bypass stent, thermal steam ablation, biological lung volume reduction, endobronchial coil placement, endobronchial occlusion and other techniques. At present, EBV is the most widely studied clinical research at home and abroad. Studies have shown highly heterogeneous emphysema. EBV is the best in patients with complete interlobar fissure and no accessory ventilation in target lobes. And Chartis evaluation system can systematically solve the above three factors. The success rate and curative effect of EBV were improved, and the development and application of HRCT and Chartis evaluation system in the treatment of severe emphysema with EBV were reviewed.
