常見婦科腫瘤血清中糖蛋白標(biāo)志物的篩選分析

【摘要】:目前,臨床上針對乳腺癌、宮頸癌、卵巢癌等常見婦科惡性腫瘤疾病的血清標(biāo)志物主要是CA153、CEA、CA125等,但在對這些腫瘤標(biāo)志物的實(shí)際臨床檢測當(dāng)中,發(fā)現(xiàn)基于這些標(biāo)志物蛋白表達(dá)水平所呈現(xiàn)出的腫瘤陽性檢出率以及特異性并不理想,用于腫瘤診斷的價(jià)值有限。尋找新的診斷標(biāo)志物成為腫瘤研究領(lǐng)域的熱點(diǎn)之一,研究表明,蛋白質(zhì)的糖基化修飾與腫瘤的發(fā)生和發(fā)展密切相關(guān)。本研究的目的就是要建立一種可通量化地篩選與鑒定血清中與婦科腫瘤疾病相關(guān)的糖蛋白的研究策略,為腫瘤的診斷標(biāo)志物以及腫瘤形成機(jī)制研究奠定基礎(chǔ)。 針對巖藻糖、唾液酸、甘露糖、N-乙酰半乳糖胺和N-乙酰葡萄糖胺等5種常見的聚糖結(jié)構(gòu)類型,本研究共選用了15種凝集素(AAL、LCA、MALⅡ、SNA、ConA、WGA、DBA、SBA、PNA、VVL、DSL、SJA、UEAⅠ、GSLⅡ、LEL)并制成凝集素芯片,通過對標(biāo)記的123例(健康對照組49例、乳腺癌組31例、宮頸癌組24例、卵巢癌組19例)臨床血清樣本的檢測,獲得全部血清樣本的聚糖結(jié)構(gòu)信息。經(jīng)過與健康對照組檢測結(jié)果的比較分析,篩選出一些在腫瘤樣本組中存在統(tǒng)計(jì)學(xué)差異的聚糖結(jié)構(gòu)信息,其中響應(yīng)信號較強(qiáng)的有:SNA(唾液酸)>ConA(甘露糖)>LCA(甘露糖)>UEA I(巖藻糖),本研究最終選用檢測信號最強(qiáng)的SNA凝集素來富集血清樣本中的唾液酸化蛋白,并用于后續(xù)唾液酸化蛋白標(biāo)志物的篩選與鑒定。 考慮到血清中高豐度蛋白的存在會(huì)對低豐度蛋白的質(zhì)譜鑒定與定量檢測產(chǎn)生嚴(yán)重干擾,在本研究中我們采取了先選擇性地去除血清樣本中兩種高豐度蛋白IgG和白蛋白,再用SNA凝集素來選擇性地富集唾液酸化糖蛋白,最后聯(lián)合iTRAQ定量標(biāo)記、HPLC預(yù)分離和Nano LC-MS/MS色譜及串聯(lián)質(zhì)譜等技術(shù)實(shí)現(xiàn)對蛋白定量分析與鑒定的研究策略,通過對乳腺癌、宮頸癌、卵巢癌和健康對照組血清樣本中的唾液酸化蛋白質(zhì)組的定量檢測分析,,共鑒定到169個(gè)蛋白質(zhì)。通過與已有文獻(xiàn)報(bào)道的研究結(jié)果相比較,可以看出本研究鑒定出的差異表達(dá)蛋白基本都是正確的,這進(jìn)一步地驗(yàn)證了本研究所建立的用于篩選疾病組血清糖蛋白標(biāo)志物的研究策略是可行的。通過與健康對照組樣本檢測結(jié)果的比較分析,在乳腺癌組發(fā)現(xiàn)30個(gè)差異表達(dá)蛋白、宮頸癌組發(fā)現(xiàn)22個(gè)差異表達(dá)蛋白、卵巢癌組發(fā)現(xiàn)30個(gè)差異蛋白。而且在不同疾病組間,這些差異蛋白在血清中的表達(dá)情況既有著相同的表達(dá)規(guī)律,又存在著疾病特異性的表達(dá)情況,為進(jìn)一步探求腫瘤疾病的發(fā)生機(jī)制以及篩選具有腫瘤特異性的標(biāo)志物奠定了基礎(chǔ)。
