高危孕婦焦慮抑郁癥狀發(fā)生率及相關(guān)危險因素（英文）

【摘要】：背景:高危孕婦與普通孕婦相比在妊娠期會出現(xiàn)更多的并發(fā)癥,高危因素作為一種應(yīng)激因素更易誘發(fā)孕婦產(chǎn)生焦慮、抑郁等負面情緒癥狀。目前國內(nèi)外對高危孕婦焦慮、抑郁癥狀的研究相對較少。目標:調(diào)查產(chǎn)科高危妊娠孕婦焦慮、抑郁癥狀發(fā)生率及其相關(guān)危險因素。方法:對197例孕中期(16-20周)的高危孕婦在妊娠最后4月每月進行綜合性醫(yī)院焦慮/抑郁量表(HAD)評估和風險因素調(diào)查。產(chǎn)后3-7天、42天及3月進行愛丁堡產(chǎn)后抑郁量表(EPDS)評估和風險因素調(diào)查。結(jié)果:入組時高危孕婦HAD焦慮均分:3.69(2.76),抑郁均分:3.42(2.53)。焦慮癥狀14例(7.18%),抑郁癥狀10例(5.13%)。多因素分析顯示,懷孕有無保胎(OR:8.162,95%CI:1.213-54.914)和乙肝陽性(OR:8.912,95%CI:1.052-75.498)與焦慮癥狀相關(guān)。尿糖陽性(OR:30.529,95%CI:1.312-710.610)和既往出血史(OR:7.122,95%CI:1.015-49.984)與抑郁癥狀相關(guān)。孕期影響高危孕婦焦慮、抑郁癥狀的因素有:近3月孕婦健康狀況、擔心胎兒健康、夫妻關(guān)系、婆媳關(guān)系等。結(jié)論:高危妊娠孕婦焦慮、抑郁癥狀較常見。近3月孕婦健康狀況、婆媳關(guān)系、夫妻關(guān)系、擔心胎兒健康是高危孕婦孕期焦慮、抑郁癥狀的風險因素。
[Abstract]:Background: compared with normal pregnant women, high-risk pregnant women will have more complications during pregnancy. As a stress factor, high-risk factors are more likely to induce anxiety, depression and other negative emotional symptoms in pregnant women. At present, there are relatively few studies on anxiety and depression in high-risk pregnant women at home and abroad. Objective: to investigate the incidence and risk factors of anxiety and depression in pregnant women at high risk of obstetrics. Methods: a total of 197 high-risk pregnant women in the second trimester of pregnancy (16 to 20 weeks) were assessed with the General Hospital anxiety / Depression scale (HAD) and risk factors in the last four months of gestation. The Edinburgh postpartum depression scale (EPDS) and risk factors were evaluated at 3 days, 42 days and 3 months after delivery. Results: the average scores of HAD anxiety and depression in high risk pregnant women were 3.69 (2.76) and 3.42 (2.53) respectively. Anxiety symptoms were 14 cases (7.18%) and depression symptoms 10 cases (5.13%). Multivariate analysis showed that pregnancy with or without pregnancy (OR:8.162,95%CI:1.213-54.914) and hepatitis B positive (OR:8.912,95%CI:1.052-75.498) were associated with anxiety symptoms. Urinary glucose positive (OR:30.529,95%CI:1.312-710.610) and past bleeding history (OR:7.122,95%CI:1.015-49.984) were associated with depressive symptoms. The factors affecting anxiety and depression in high-risk pregnant women during pregnancy include: maternal health status in recent three months, fear of fetal health, marital relationship, relationship between mother-in-law and daughter-in-law, and so on. Conclusion: anxiety and depression are more common in high-risk pregnant women. In recent 3 months, the health status of pregnant women, the relationship between mother-in-law and daughter-in-law, the relationship between husband and wife, and the fear of fetal health are the risk factors of anxiety and depression in high risk pregnant women.
