【摘要】：目的：本文通過對(duì)針刺聯(lián)合克羅米芬治療黃體功能不足性不孕癥進(jìn)行臨床療效觀察,并與單純克羅米芬組予以對(duì)照并且進(jìn)行客觀評(píng)價(jià),積極探索針刺在治療本病中的作用及機(jī)理,明確針刺具有改善卵巢功能,促進(jìn)卵泡發(fā)育,提高黃體功能的重要作用。為治療因黃體功能不足所引起的不孕癥提出安全有效的臨床治療方案。 方法：按照臨床研究程序和方法,搜集符合納入標(biāo)準(zhǔn)的因黃體功能不足導(dǎo)致不孕的患者36例,隨機(jī)分為兩組：治療組18例應(yīng)用針刺聯(lián)合克羅米芬治療,對(duì)照組18例單純應(yīng)用克羅米芬進(jìn)行治療。觀察兩組患者經(jīng)過三個(gè)月經(jīng)周期治療前后的妊娠情況、中醫(yī)證候、卵泡發(fā)育、子宮內(nèi)膜情況、BBT、血清雌孕激素含量的改善情況及BMI的變化情況。經(jīng)療效評(píng)分與統(tǒng)計(jì)學(xué)方法處理,客觀的評(píng)價(jià)針刺聯(lián)合克羅米芬治療本病的臨床療效。 結(jié)果：經(jīng)過針刺聯(lián)合藥物治療后黃體功能不足性不孕患者有7例妊娠,妊娠率為41.18%,西藥組5例妊娠,妊娠率為29.41%,比較無統(tǒng)計(jì)學(xué)意義(P0.05),但治療組優(yōu)于對(duì)照組；兩組間比較治療組在改善中醫(yī)臨床癥狀和基礎(chǔ)體溫的情況上明顯優(yōu)于對(duì)照組(P0.05)。在改善血清雌、孕激素、卵泡最大直徑、子宮內(nèi)膜厚度的對(duì)比中治療組與對(duì)照組都具有統(tǒng)計(jì)學(xué)意義(P0.05),治療組明顯好于對(duì)照組。在體重指數(shù)的對(duì)照中,兩組的比較無明顯差異(P0.05)。 結(jié)論：針刺具有改善卵巢的黃體功能,促進(jìn)卵泡的發(fā)育,調(diào)整月經(jīng)周期和提高血清雌、孕激素含量,增加妊娠率和降低早期流產(chǎn)率的作用,安全有效,患者無不良反應(yīng),值得進(jìn)一步研究推廣。
[Abstract]:Objective: To observe the effect and mechanism of acupuncture combined with Kuromifene in the treatment of infertility due to the deficiency of the corpus luteum, and to compare with the simple Kuromifene group and to carry out the objective evaluation, and to explore the role and mechanism of acupuncture in the treatment of the disease. It has an important role in improving the ovarian function, promoting the development of the follicle, and improving the function of the corpus luteum. And provides a safe and effective clinical treatment scheme for treating the infertility caused by the insufficiency of the corpus luteum function. Methods: According to the clinical study procedure and method,36 patients with infertility due to the deficiency of the corpus luteum were collected and randomly divided into two groups. The status of pregnancy, the syndrome of TCM, the development of the follicle, the condition of the endometrium, the BBT, the level of the estrogen and the progestogen in the serum and the change of the BMI were observed. Objective To evaluate the clinical treatment of the treatment of this disease by means of the treatment of the curative effect score and the statistical method and the objective evaluation of the combination of the acupuncture combined with the Kuromiphene. Results: There were 7 cases of pregnancy, the pregnancy rate was 41.18%, and the pregnancy rate was 29.41% in the western medicine group, compared with that of the western medicine group (P <0.05), but the treatment group was superior to that of the treatment group. In the control group, the comparison between the two groups was superior to that of the control group (P0) in the case of improving the clinical symptoms and basal body temperature of the Chinese medicine. (05) There was a significant difference between the treatment group and the control group (P <0.05) in the treatment group and the control group after the improvement of the serum estrogen, the progestogen, the maximum diameter of the follicle and the thickness of the endometrium (P0.05). Conclusion: Acupuncture has the function of improving the corpus luteum function of the ovary, promoting the development of the follicle, regulating the menstrual cycle and improving the female and progestogen content in the serum, increasing the pregnancy rate and reducing the early abortion rate.
