【摘要】：目的運用三維能量多普勒超聲定量檢測多囊卵巢綜合征(PCOS)胰島素抵抗(IR)患者卵巢間質血流指數(shù)及相關參數(shù),探討二甲雙胍PCOS-IR患者卵巢間質微循環(huán)血流的影響。方法選取PCOS-IR患者40例為觀察組,正常排卵的已婚女性20例為對照組,觀察組服用二甲雙胍共3個周期。比較觀察組與對照組及觀察組治療前后卵巢間質血流參數(shù)(VI、FI、VFI)、卵巢容積、卵泡數(shù)目及生殖內分泌指標的差異。結果觀察組較對照組卵巢間質血流參數(shù)升高,觀察組治療3個月后卵泡數(shù)目減少,卵巢間質血流參數(shù)VI、FI、VFI降低、生殖內分泌LH、LH/FSH、T較治療前降低,差異均有統(tǒng)計學意義(P0.05);卵巢容積減小,差異無統(tǒng)計學意義(P0.05)。結論二甲雙胍可以改善PCOS-IR患者卵巢間質血流微循環(huán)狀況,三維能量多普勒超聲能為PCOS的治療效果的評判提供卵巢血流動力學量化方案,具有一定臨床價值。
[Abstract]:Objective to investigate the effect of metformin PCOS-IR on interstitial blood flow in patients with polycystic ovary syndrome (PCOS) with insulin resistance (IR) by using three dimensional power Doppler ultrasound. Methods 40 patients with PCOS-IR and 20 married women with normal ovulation were selected as observation group and control group. The observation group took metformin for 3 cycles. The difference of ovarian interstitial blood flow parameters (VI,FI,VFI), ovarian volume, follicle number and reproductive endocrine index before and after treatment were compared between the observation group and the control group and the observation group. Results compared with the control group, the blood flow parameters of the ovarian stroma in the observation group increased, the number of follicles decreased, the VI,FI,VFI of the interstitial blood flow parameters of the ovary decreased, and the reproductive endocrine LH,LH/FSH,T decreased after 3 months of treatment in the observation group. The difference was statistically significant (P0.05). Ovarian volume decreased, the difference was not statistically significant (P0.05). Conclusion metformin can improve the microcirculation of ovarian stroma in patients with PCOS-IR. Three-dimensional power Doppler ultrasound can provide a quantitative scheme of ovarian hemodynamics for evaluating the therapeutic effect of PCOS, and it has certain clinical value.
