29例剖宮產(chǎn)手術(shù)切口感染的回顧性分析及預(yù)防策略

【摘要】:目的探討剖宮產(chǎn)手術(shù)切口感染的危險因素,為有效預(yù)防和控制剖宮產(chǎn)手術(shù)切口感染提供科學依據(jù)。方法采用回顧性調(diào)查方法,2011年12月至2012年12月在該院實施剖宮產(chǎn)手術(shù)發(fā)生切口感染的29例產(chǎn)婦作為觀察組,抽取同期未發(fā)生切口感染的29例產(chǎn)婦作為對照組,比較兩組各方面的差異,確定切口感染的危險因素。結(jié)果 589例剖宮產(chǎn)中有29例發(fā)生切口感染,感染率為4.9%。單因素分析結(jié)果顯示,觀察組產(chǎn)婦BMI(25.9±1.1)kg/m2,疤痕子宮6例,急診手術(shù)18例,手術(shù)時間(1.2±0.6)h,住院時間(7.1±2.3)d,合并基礎(chǔ)疾病為6例,胎膜早破為12例,陰道及肛診次數(shù)(3.3±0.6)次,與對照組各項比較,差異有統(tǒng)計學意義(P0.05)。多因素分析結(jié)果顯示,BMI(OR=2.802)、手術(shù)時間(OR=5.813)、合并基礎(chǔ)疾病(OR=6.182)、胎膜早破(OR=11.219)為剖宮產(chǎn)手術(shù)切口感染的獨立危險因素。結(jié)論多種因素影響手術(shù)切口感染,對高危人群采取針對性的預(yù)防控制措施,從而降低剖宮產(chǎn)手術(shù)切口感染率。
[Abstract]:Objective to explore the risk factors of incision infection in cesarean section and to provide scientific basis for prevention and control of incision infection. Methods from December 2011 to December 2012, 29 pregnant women who had incisional infection during cesarean section operation in our hospital were selected as observation group and 29 women without incision infection as control group. The risk factors of incision infection were determined by comparing the differences between the two groups. Results among 589 cases of cesarean section, 29 cases had incision infection, the infection rate was 4.9%. Univariate analysis showed that BMI (25.9 鹵1.1) kg/m2, scar uterus in 6 cases, emergency operation in 18 cases, operation time (1.2 鹵0.6) h, hospital stay (7.1 鹵2.3) d, and basic diseases in 6 cases. 12 cases of premature rupture of membranes, vaginal and anal diagnosis times (3.3 鹵0.6), compared with the control group, the difference was statistically significant (P0.05). Multivariate analysis showed that, BMI (OR=2.802, OR=5.813, OR=6.182 and OR=11.219 were independent risk factors for incision infection in cesarean section. Conclusion the infection rate of incision in cesarean section can be reduced by many factors, and the prevention and control measures should be taken to the high risk population in order to reduce the infection rate of incision in cesarean section.
