【摘要】：目的:比較宮頸機(jī)能不全患者非孕期行腹腔鏡下子宮峽部環(huán)扎術(shù)與經(jīng)陰道子宮峽部環(huán)扎術(shù)的治療效果。方法:回顧性分析2013年5月至2015年5月于我院確診為宮頸機(jī)能不全的非孕患者,接受腹腔鏡或經(jīng)陰道子宮峽部環(huán)扎術(shù)后5月內(nèi)自然受孕,單胎并隨訪資料完整的病例共計(jì)63例,其中接受腹腔鏡下子宮峽部環(huán)扎術(shù)患者30例(研究組),經(jīng)陰道子宮峽部環(huán)扎術(shù)患者33例(對照組),對兩種手術(shù)方式的臨床效果進(jìn)行比較。結(jié)果:研究組治療后孕周≥28周的成功率(96.67%)顯著高于對照組(51.52%)(P0.05),研究組術(shù)后平均分娩孕周較對照組延長(P0.05),研究組的流產(chǎn)率(3.33%)、早產(chǎn)率(10.00%)較對照組低(48.48%,30.30%),足月產(chǎn)率(86.67%)較對照組高(21.21%),差異有統(tǒng)計(jì)學(xué)意義(P0.05)。研究組患者手術(shù)時(shí)間(37.27±1.93分鐘)及住院時(shí)間(5.17±0.38天)短于對照組(52.30±2.36分鐘,8.24±2.22天),術(shù)中出血量(13.13±1.57 ml)少于對照組(31.61±1.87 ml),差異有統(tǒng)計(jì)學(xué)意義(P0.05)。研究組與對照組術(shù)后并發(fā)癥發(fā)生率分別為0、18.18%,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:非孕期腹腔鏡下子宮峽部環(huán)扎術(shù)治療宮頸機(jī)能不全相比經(jīng)陰道子宮峽部環(huán)扎術(shù)臨床治療效果更好,具有較高的安全性,值得臨床推廣。
[Abstract]:Objective: to compare the effect of laparoscopic uterine isthmus ligation and transvaginal uterine isthmus ligation in non-pregnant patients with cervical insufficiency.
