維生素D對妊娠糖尿病患者胰島素敏感性及脂肪因子APN、RPB4、Visfatin及TNF-α的作用機(jī)制研究

【摘要】:目的:探討血清維生素D水平與妊娠糖尿。℅DM)患者胰島素敏感性及對脂肪因子脂聯(lián)素(APN)、視黃醇結(jié)合蛋白4(RBP4)、內(nèi)脂素(Visfatin)及腫瘤壞死因子(TNF-α)的影響及作用機(jī)制。方法:選擇101例妊娠糖尿病患者(GDM組)和50例血糖正常孕婦(對照組),檢測二組檢測對象相關(guān)生化指標(biāo)、APN、RBP4、Visfatin、TNFα及25-(OH)D3,之后在診斷為低維生素D患者中分別隨機(jī)取12例GDM治療組及12例控制組,補(bǔ)充1月維生素D再行相關(guān)生化指標(biāo)檢查。結(jié)果:GDM組患者測定25-(OH)D3及APN水平明顯低于對照組(P<0.05),而RBP4、Visfatin、TNF-α水平高于對照組(均P<0.05);經(jīng)補(bǔ)充維生素D約1月,,胰島素分泌指數(shù)上升,胰島素抵抗指數(shù)下降,而APN水平升高,與25-(OH)D3呈正相關(guān)(r值0.526); RBP4、Visfatin、TNF-α降低,與25-(OH)D3呈負(fù)相關(guān)(r值分別為-0.272,-0.153,-0.072)。結(jié)論:維生素D可通過降低相關(guān)脂肪因子RBP4、Visfatin、TNF-α,升高APN,從而對妊娠糖尿病起到保護(hù)作用。
[Abstract]:Objective: to investigate the relationship between serum vitamin D level, insulin sensitivity and adiponectin (APN), retinol binding protein 4 (RBP4) in patients with gestational diabetes mellitus (GDM). The effect and mechanism of (Visfatin) and TNF- 偽. Methods: 101 patients with gestational diabetes mellitus (GDM group) and 50 normal pregnant women with normal blood glucose (control group) were selected. The biochemical indexes, APN,RBP4,Visfatin,TNF 偽 and 25 (OH) D3 were detected in the two groups. Then 12 cases of GDM treatment group and 12 cases of control group were randomly selected in the diagnosis of low vitamin D. Results: the levels of 25- (OH) D3 and APN in GDM group were significantly lower than those in control group (P < 0. 05), while RBP4,Visfatin,TNF- 偽 level was higher than that in control group (all P < 0. 05). After vitamin D supplementation for about one month, insulin secretion index increased, insulin resistance index decreased, and APN level increased, which was positively correlated with 25- (OH) D3 (r = 0.526). The decrease of RBP4,Visfatin,TNF- 偽 was negatively correlated with 25 (OH) D3 (r = -0.272 ~ -0.153 ~ 0. 072). Conclusion: vitamin D can protect gestational diabetes by decreasing RBP4,Visfatin,TNF- 偽 and increasing APN,.
