【摘要】：目的:探討雙胎未足月胎膜早破(PPROM)的臨床特點和妊娠結(jié)局。方法:回顧性分析雙胎PPROM122例(研究組)及同期隨機(jī)抽取的單胎PPROM183例(對照組),探討兩組胎膜早破發(fā)生率、發(fā)病孕周、期待治療、新生兒預(yù)后等情況。結(jié)果:雙胎妊娠PPROM發(fā)生率為21.0%,顯著高于單胎妊娠PPROM發(fā)生率(3.5%)(P0.05);研究組早產(chǎn)兒體重明顯低于對照組,窒息率明顯高于對照組(P0.05);研究組胎膜早破孕周、終止妊娠孕周均早于對照組,研究組34周發(fā)生胎膜早破者期待時間明顯短于對照組(P0.05)。結(jié)論:雙胎妊娠易發(fā)生胎膜早破、早產(chǎn)、新生兒窒息,期待治療時間短,應(yīng)積極預(yù)防,出現(xiàn)胎膜早破積極對癥治療,選擇合適的分娩方式,改善新生兒結(jié)局。
[Abstract]:Objective: to investigate the clinical features and pregnancy outcome of unmatured premature rupture of membranes (PPROM) in twins. Methods: the cases of twin PPROM122 (study group) and single fetal PPROM183 (control group) were analyzed retrospectively. The incidence of premature rupture of membranes, gestational weeks, expectant treatment and prognosis of newborns were studied. Results: the incidence of PPROM in twin pregnancy was 21.0, which was significantly higher than that in single pregnancy (3.5%), the weight of premature infants in study group was significantly lower than that in control group, and the asphyxia rate in study group was significantly higher than that in control group (P0.05). The expected time of premature rupture of membranes in the study group was significantly shorter than that in the control group at 34 weeks (P0.05). Conclusion: premature rupture of membranes preterm delivery and asphyxia of newborn are easy to occur in twin pregnancy. It is expected that the treatment time is short and should be actively prevented. The occurrence of premature rupture of membranes should be treated positively and the proper delivery mode should be selected to improve the neonatal outcome.
