稽留流產(chǎn)的胎兒絨毛染色體異常及相關(guān)因素分析

【摘要】:目的:探討稽留流產(chǎn)的胚胎絨毛染色體異常與口服葉酸、年齡、胚胎孕周及流產(chǎn)次數(shù)的關(guān)系。方法:收集稽留流產(chǎn)的胚胎絨毛組織82例,采用新一代基因測(cè)序(NGS)技術(shù)檢測(cè)胚胎絨毛染色體,分析口服葉酸、年齡、胚胎孕周、流產(chǎn)次數(shù)與染色體異常的關(guān)系。結(jié)果:82例稽留流產(chǎn)的胚胎絨毛染色體中,46例(56.10%,46/82)異常,其中數(shù)目異常38例(82.61%,38/46),結(jié)構(gòu)異常8例(17.39%,8/46),三體發(fā)生率89.47%(34/38)。82例胚胎絨毛染色體中,排除胚胎孕周等因素對(duì)年齡的影響,選擇25~39歲孕婦的胚胎絨毛染色共74例,其中染色體異常率62.16%(46/74),35~39歲孕婦的染色體異常率(43.48%,20/46)高于30~34歲(23.91%,11/46)、25~30歲(32.61%,15/46),差異有統(tǒng)計(jì)學(xué)意義(P0.05),30~35歲孕婦的染色體異常率與25~30歲孕婦比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。82例胚胎絨毛染色體中,排除年齡等因素對(duì)胚胎孕周的影響,胚胎停育孕周8~12周共78例,染色體異常率55.13%(43/78),胚胎停育孕周≤8周的染色體異常率(58.14%,25/43)高于8~12周(41.86%,18/43),差異有統(tǒng)計(jì)學(xué)意義(P0.0167)。流產(chǎn)1~5次的染色體異常率與流產(chǎn)0次比較,口服葉酸與未口服葉酸比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:胚胎絨毛染色體異常是引起稽留流產(chǎn)的重要因素,稽留流產(chǎn)的胚胎絨毛染色體異常與年齡、胚胎孕周密切相關(guān),與是否口服葉酸、流產(chǎn)次數(shù)(0~5次)無(wú)顯著關(guān)系。
