人乳頭狀瘤病毒基因分型與宮頸非典型腺細胞檢測的臨床研究

【摘要】:目的了解宮頸存在非典型腺細胞時人乳頭狀瘤病毒(HPV)感染和基因分布,為進一步診斷和判斷預后提供依據。方法回顧性分析2012年4月-2013年3月因宮頸存在非典型腺細胞的53例婦女為觀察組,平均年齡57歲;宮頸無上皮內病變或惡性病變的338名婦女為對照組,平均年齡62歲,所有研究對象均進行宮頸液基細胞檢測(TCT)、HPV-DNA分型。結果觀察組有34.0%感染≥1種高危型HPV(HR-HPV),HPV-16和(或)HPV-18感染率為20.8%,與對照組比較,差異有統計學意義(P0.05);觀察組單一HPV亞型感染率為40.0%、多重HPV亞型感染率為9.5%、多重HR-HPV感染率為5.7%,以單一HPV亞型感染更為常見,差異有統計學意義(P0.05);兩組間低危型HPV(LR-HPV)感染率比較為13.2%與17.2%,差異無統計學意義。結論宮頸存在非典型腺細胞時有較高的HR-HPV感染率,HR-HPV基因檢測及分型對宮頸存在非典型腺細胞的預后判斷、療效監(jiān)測等具有重要價值。
[Abstract]:Objective to investigate the (HPV) infection and gene distribution of human papillomavirus (HPV) in the presence of atypical adenocytes in the cervix, so as to provide evidence for further diagnosis and prognosis. Methods A retrospective analysis of 53 women with atypical glandular cells in the cervix from April 2012 to March 2013 was performed, with an average age of 57 years, and 338 women with no intraepithelial or malignant cervical lesions as control group, with an average age of 62 years. All subjects were tested for (TCT), HPV-DNA typing by cervical fluid-based cells. Results in the observation group, 34.0% of the patients were infected with more than one high-risk type of HPV (HR-HPV), and the infection rate of HPV-16 and / or HPV-18 was 20.8%, compared with the control group. The infection rate of single HPV subtype was 40.0 in the observation group, 9.5in the multiple HPV subtype, 5.7 in the multiple HR-HPV infection rate, especially in the single HPV subtype infection. The infection rate of low risk HPV (LR-HPV) between the two groups was 13. 2% and 17. 2% respectively. There was no significant difference between the two groups. Conclusion there is a high rate of HR-HPV infection in the presence of atypical glandular cells in the cervix. The detection and typing of HR-HPV gene is of great value for the prognosis of atypical glandular cells in the cervix and the monitoring of the curative effect.
