【摘要】：目的:研究子宮靜脈內(nèi)平滑肌瘤病的臨床病理特征,探討該疾病的診斷和治療策略。方法:回顧分析2007年7月至2013年6月復(fù)旦大學(xué)附屬婦產(chǎn)科醫(yī)院收治的64例靜脈內(nèi)平滑肌瘤病患者的臨床病理資料。結(jié)果:子宮靜脈內(nèi)平滑肌瘤病的臨床表現(xiàn)缺乏特異性,與普通子宮平滑肌瘤類似,主要為月經(jīng)量增多、腹痛腹脹、盆腔腫塊及繼發(fā)貧血。僅有3例在術(shù)前彩色多普勒超聲檢查中被疑診為子宮靜脈內(nèi)平滑肌瘤病。術(shù)中冰凍病理多能提示靜脈內(nèi)平滑肌瘤病,肌瘤主要位于子宮體(73.4%)、闊韌帶(18.8%)及子宮峽部(7.8%)。手術(shù)方式包括子宮肌瘤剝出術(shù)(4.7%)、全子宮切除術(shù)(39.1%)及全子宮+雙附件切除術(shù)(56.2%)。術(shù)后獲訪的50例患者的隨訪中位時間為28個月,僅有1例在子宮肌瘤剝出術(shù)后復(fù)發(fā),行全子宮切除術(shù)后隨訪至今未復(fù)發(fā)。結(jié)論:子宮靜脈內(nèi)平滑肌瘤病術(shù)前診斷困難,手術(shù)切除是主要治療方式。術(shù)中仔細(xì)探查,選擇適當(dāng)?shù)氖中g(shù)方式可獲得滿意的治療效果,術(shù)后需密切隨訪。
[Abstract]:Objective: to study the clinicopathological features of intrauterine leiomyomatosis and to explore the diagnosis and treatment of the disease. Methods: the clinicopathological data of 64 patients with venous leiomyomatosis admitted to the Department of Obstetrics and Gynecology of Fudan University from July 2007 to June 2013 were retrospectively analyzed. Results: the clinical manifestations of leiomyomatosis in uterine vein were lack of specificity and were similar to those of normal uterine leiomyoma. The main clinical manifestations were increased menstrual volume abdominal pain abdominal distension pelvic mass and secondary anemia. Only 3 cases were suspected of intrauterine leiomyomatosis by color Doppler ultrasonography before operation. The intraoperative frozen pathology showed that the leiomyomatosis of vein was mainly located in uterine body (73.4%), broad ligament (18.8%) and uterine isthmus (7.8%). The operative methods included myomectomy (4.7%), total hysterectomy (39.1%) and total double appendage hysterectomy (56.2%). The median follow-up time of 50 cases was 28 months. Only 1 case recurred after myomectomy, and no recurrence occurred after hysterectomy. Conclusion: preoperative diagnosis of intrauterine leiomyomatosis is difficult and surgical resection is the main treatment. Careful exploration during operation and selection of appropriate surgical methods can obtain satisfactory therapeutic effect and should be followed up closely after operation.
