微量子宮內(nèi)膜活檢在子宮內(nèi)膜癌及癌前病變篩查中的應(yīng)用價(jià)值

【摘要】:目的評估微量子宮內(nèi)膜活檢標(biāo)本在子宮內(nèi)膜癌及癌前病變篩查中的應(yīng)用價(jià)值。方法收集北京大學(xué)人民醫(yī)院及北京市大興區(qū)人民醫(yī)院子宮內(nèi)膜活檢標(biāo)本589例,分別采用環(huán)狀微量子宮內(nèi)膜取樣器以及分段診刮獲取子宮內(nèi)膜組織,進(jìn)行病理學(xué)診斷,觀察微量子宮內(nèi)膜活檢取樣合格率,及其與子宮內(nèi)膜厚度、年齡的關(guān)系,并分析微量子宮內(nèi)膜活檢在子宮內(nèi)膜癌及癌前病變篩查中的敏感度及特異性,篩選其相對適用人群。結(jié)果兩家醫(yī)院微量子宮內(nèi)膜活檢取樣合格率為71.5%(421/589),子宮內(nèi)膜較厚(≥5 mm)、年齡較小(50歲)的患者更易獲得合格的微量子宮內(nèi)膜活檢標(biāo)本;在微量子宮內(nèi)膜活檢取樣合格的標(biāo)本中檢出子宮內(nèi)膜癌及癌前病變55例,與分段診刮比較其敏感性為75.3%(55/73),特異性為99.4%(332/334)。結(jié)論微量子宮內(nèi)膜活檢標(biāo)本合格率仍有待提高,相對適用于年輕及子宮內(nèi)膜較厚的人群,但在取樣合格的標(biāo)本中,其篩查子宮內(nèi)膜癌及癌前病變特異度較高,表明采用微量子宮內(nèi)膜活檢可以作為高危人群子宮內(nèi)膜癌篩查的備選方法。
[Abstract]:Objective to evaluate the value of micro endometrial biopsy specimens in screening of endometrial carcinoma and precancerous lesions. Methods 589 specimens of endometrium biopsy specimens from Peking University People's Hospital and hospital were collected, and the endometrium tissue was obtained by circular micro endometrium sampler and segmental curettage. The qualification rate of microendometrial biopsy samples, the relationship with endometrial thickness and age, and the sensitivity and specificity of microendometrial biopsy in endometrial cancer and precancerous lesions were analyzed and the relative population was screened by microendometrial biopsy. The results were 71.5% (42). 1/589), the endometrium was thicker (more than 5 mm), and the younger (50 years old) patients were more likely to obtain qualified micro endometrium biopsy specimens; 55 cases of endometrial cancer and precancerous lesions were detected in qualified specimens of micro endometrial biopsy samples. Compared with the segmental curettage, the sensitivity was 75.3% (55/73) and the specificity was 99.4% (332/334). The qualified rate of endometrium biopsy remains to be improved, which is relatively suitable for young people and thicker endometrium, but the specificity of screening for endometrial cancer and precancerous lesions is high in qualified samples, indicating that micro endometrial biopsy can be used as a alternative for screening endometrium cancer in high-risk groups.
