【摘要】：目的探討舒芬太尼替代芬太尼在無(wú)痛分娩應(yīng)用的安全性和可行性。方法將2014年8月-2016年2月在我院接受自然生產(chǎn)的初產(chǎn)婦300例,按患者入院的先后順序及意愿選擇分娩方式分為芬太尼組、舒芬太尼組和對(duì)照組3組,每組100例。對(duì)照組自然生產(chǎn),芬太尼組、舒芬太尼組分別采用芬太尼和舒芬太尼無(wú)痛分娩,產(chǎn)程若出現(xiàn)宮縮乏力加滴縮宮素。觀察總產(chǎn)程時(shí)間,視覺(jué)模擬評(píng)分(visual analogue scale,VAS)進(jìn)行疼痛評(píng)分,并發(fā)癥,新生兒Apgar 1、10 min評(píng)分。結(jié)果總產(chǎn)程時(shí)間對(duì)照組明顯短于芬太尼組[(402.8±81.9)min vs.(476.5±84.8)min,q=9.185,P0.05];疼痛VAS評(píng)分舒芬太尼組、芬太尼組明顯低于對(duì)照組[(3.0±0.7)分vs.(7.1±0.7)分,q=58.571,P0.05;(3.3±0.7)分vs.(7.1±0.7)分,q=54.286,P0.05]。3組產(chǎn)后并發(fā)癥發(fā)生率無(wú)統(tǒng)計(jì)學(xué)差異(χ~2=0.150,P=0.928)。3組新生兒Apgar 1、10 min評(píng)分有統(tǒng)計(jì)學(xué)差異(P0.05),但都在8分以上,差異無(wú)實(shí)際臨床意義。結(jié)論舒芬太尼應(yīng)用于無(wú)痛分娩時(shí)聯(lián)合縮宮素能獲得滿意的鎮(zhèn)痛效果,安全有效。
[Abstract]:Objective to investigate the safety and feasibility of sufentanil replacing fentanyl in painless delivery. Methods from August 2014 to February 2016, 300 primiparas who were given birth in our hospital were divided into three groups: fentanyl group, sufentanil group and control group, 100 cases in each group. Control group, fentanyl group and sufentanil group were given fentanyl and sufentanil painless delivery respectively. The total duration of labor, visual analogue score (visual analogue scale), pain score, complications and neonatal Apgar 1 + 10 min score were observed. Results the total duration of labor in the control group was significantly shorter than that in the fentanyl group [(402.8 鹵81.9) min vs. (鹵84.8) min, P 0.05], and the pain score in sufentanil group was significantly lower than that in the fentanyl group. The incidence of postpartum complications in fentanyl group was significantly lower than that in control group [(3.0 鹵0.7) scores vs. (7.1 鹵0.7) scores vs 58.571g P0.05; (3.3 鹵0.7) scores vs 54.286p0.05] .3, there was no statistical difference in the incidence of postpartum complications (蠂 2 0.150 P 0.928). Conclusion Sufentanil combined with oxytocin in painless delivery can obtain satisfactory analgesic effect and is safe and effective.
