【摘要】：【目的】：系統(tǒng)評(píng)價(jià)保留生育功能手術(shù)治療卵巢交界性腫瘤(Borderline ovarian tumor，BOT)的生存率、復(fù)發(fā)率和術(shù)后妊娠情況。 【方法】：計(jì)算機(jī)全面檢索Pubmed、Medline、embase數(shù)據(jù)庫等外文及CNKI、維普、萬方、中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫等中文數(shù)據(jù)庫，檢索1990年1月1日至2014年3月1日期間發(fā)表的中英文文獻(xiàn)，，對(duì)得到的文獻(xiàn)分別進(jìn)行評(píng)價(jià)、提取、剝除/引用、核對(duì)后，將納入符合標(biāo)準(zhǔn)的前瞻性病例對(duì)照研究、回顧性病例對(duì)照研究，對(duì)同質(zhì)性研究采用RevMan5.2.7軟件進(jìn)行Meta分析。 【結(jié)果】：共納入14篇保留生育手術(shù)和非保留生育功能手術(shù)治療卵巢交界性腫瘤生存率、復(fù)發(fā)率、生育情況比較的病例對(duì)照研究，共納入患者2244例，其中948例（42%）行保留生育功能手術(shù)，14篇研究中1篇為前瞻性病例對(duì)照研究，13篇為回顧性回顧性病例對(duì)照研究。Meta分析結(jié)果表明，卵巢交界性腫瘤保留生育手術(shù)患者與非保留生育功能手術(shù)患者總體生存率增高，差異有統(tǒng)計(jì)學(xué)意義（OR=2.15，95%CI（1.09,4.23），P=0.03）。兩組FIGOⅠ期患者生存率無明顯差別，無統(tǒng)計(jì)學(xué)意義（OR=1.86，95%CI為（0.57,6.06），P=0.3）。保留生育功能手術(shù)復(fù)發(fā)率高于非保留生育功能手術(shù)，具有顯著性統(tǒng)計(jì)學(xué)差異（OR=3.85，95%CI為（2.62,5.68），且P＜0.00001）。9篇臨床病例對(duì)照研究記錄了保留生育功能手術(shù)后的生育情況，其中4篇文獻(xiàn)詳細(xì)記錄、分析了有生育要求并嘗試懷孕和實(shí)際成功受孕患者的例數(shù)，并計(jì)算出總的妊娠率為55.6%。 【結(jié)論】保留生育功能手術(shù)雖然能增加卵巢交界性腫瘤患者的復(fù)發(fā)率，但不會(huì)降低患者的生存率，且維持了正常的妊娠、生育功能，增加了患者對(duì)生活的信心、提高了生存質(zhì)量。
[Abstract]:[objective]: to evaluate systematically the survival rate, recurrence rate and postoperative pregnancy status of borderline ovarian tumor and BOT.Methods: a comprehensive computer search was conducted for Pubmedan Medlineembase database, CNKI, Weip, Wanfang, etc. The Chinese biomedical literature database and other Chinese databases were used to retrieve Chinese and English documents published between January 1, 1990 and March 1, 2014. The obtained documents were evaluated, extracted, stripped / quoted, and checked. Will include prospective case-control studies that meet the criteria, retrospective case-control studies, Meta-analysis was performed with Revman 5.2.7 software for homogeneity study.
