【摘要】：目的了解河北省承德市女性宮頸人乳頭瘤病毒(HPV)感染情況,分析HPV感染的危險因素,為HPV感染及宮頸癌防治提供理論依據(jù)。方法于2013年1月—2016年1月在承德市8縣3區(qū)選取4 250名女性進行問卷調(diào)查、液基薄層細(xì)胞學(xué)檢查(TCT)及宮頸HPV DNA檢測,對HPV16/18型和細(xì)胞學(xué)提示不能明確意義的非典型鱗狀上皮細(xì)胞(ASCUS)、非典型腺上皮細(xì)胞(AGC)及以上患者進行陰道鏡檢查、組織活檢及病理檢查,分析不同宮頸病變及年齡組HPV感染情況,采用多因素logistic回歸模型分析HPV感染危險因素。結(jié)果承德地區(qū)4 250名婦女HPV感染819例,感染率為19.27%,最常見的5種HPV高危型別依次為16、58、52、39、51型。伴隨宮頸上皮內(nèi)瘤變(CIN)級別升高,HPV感染率升高,宮頸癌HPV感染率為100%。20歲組HPV感染率最高,為21.74%;30~49歲及≥60歲組為HPV感染高峰。吸煙、首次性生活年齡過早、受教育程度低(高中以下)、接觸性出血和宮頸柱狀上皮異位為HPV感染的危險因素,避孕套避孕和了解宮頸癌及HPV為HPV感染的保護因素。結(jié)論HPV感染與CIN及宮頸癌密切相關(guān),加強對具有HPV感染危險因素人群的篩查及管理可有效預(yù)防宮頸癌的發(fā)生。
[Abstract]:Objective to investigate the infection of cervical human papillomavirus (HPV) in women in Chengde, Hebei Province, and to analyze the risk factors of HPV infection, and to provide theoretical basis for the prevention and treatment of HPV infection. Methods from January 2013 to January 2016, 4 250 women were selected from 8 counties and 3 districts in Chengde City to carry out questionnaire survey, liquid based thin layer cytology examination (TCTT) and cervical HPV DNA detection. The patients with atypical squamous epithelial cells (ASSCS) or atypical glandular epithelial cells (AGC) or above whose HPV16/18 and cytology were not clear were examined by colposcopy, biopsy and pathological examination. The infection of HPV in different cervical lesions and age groups was analyzed. Multivariate logistic regression model was used to analyze the risk factors of HPV infection. Results there were 819 cases of HPV infection among 4 250 women in Chengde area, the infection rate was 19.27%. The most common high risk type of HPV was 1658A52P39C51. With the increase of the grade of cervical intraepithelial neoplasia (cin), the infection rate of HPV was increased.
