【摘要】：宮頸上皮內(nèi)瘤變(CIN)是與宮頸癌密切相關(guān)的癌前病變。隨著宮頸癌生物學(xué)研究的進(jìn)展和篩查技術(shù)的進(jìn)步,宮頸癌的篩查目標(biāo)已轉(zhuǎn)變?yōu)樵缙跈z出高級別CIN并行阻斷性治療。在CIN治療策略的原則中,首要目的應(yīng)是預(yù)防宮頸癌的發(fā)生。目前CIN的診斷仍依靠宮頸細(xì)胞學(xué)、陰道鏡與組織病理學(xué)這三階梯技術(shù)("three step"technique)。治療方面,推薦期待治療作為CINⅠ女性的標(biāo)準(zhǔn)治療。對于CINⅡ與CINⅢ,宮頸環(huán)形電切術(shù)(LEEP)和激光錐切是推薦的治療方法。術(shù)后主要通過細(xì)胞學(xué)、人乳頭瘤病毒(HPV)檢測對低至中度疾病殘留及復(fù)發(fā)風(fēng)險患者進(jìn)行隨訪,對高度疾病殘留及復(fù)發(fā)風(fēng)險患者,推薦陰道鏡隨診。現(xiàn)擬從CIN的診斷、轉(zhuǎn)歸、治療、CIN合并妊娠、CIN治療后對妊娠結(jié)局的影響以及隨訪等方面進(jìn)行綜述。
[Abstract]:Cervical intraepithelial neoplasia (cin) is a precancerous lesion closely related to cervical cancer. With the progress of biological research and screening technology of cervical cancer, the goal of cervical cancer screening has been transformed into the early detection of high-level CIN and blocking therapy. In the principle of CIN treatment strategy, the primary purpose should be to prevent the occurrence of cervical cancer. At present, the diagnosis of CIN still depends on the three-step technique of cervix cytology, colposcopy and histopathology ("three step" technique).
