【摘要】：目的探討宮腔鏡下活檢、診斷性刮宮兩種術(shù)前病理檢查方法聯(lián)合血清CA125在子宮癌肉瘤診斷中的價值。方法收集2000年1月至2012年1月廣東省子宮內(nèi)膜癌防治項目資料相對完整,經(jīng)手術(shù)病理確診的子宮癌肉瘤患者106例,對其術(shù)前檢查方法進(jìn)行回顧性分析。結(jié)果 (1)術(shù)前行診斷性刮宮74例,組織病理學(xué)報告為子宮癌肉瘤、其他子宮惡性腫瘤、子宮內(nèi)膜不典型增生分別占35.1%、52.7%、9.5%;宮腔鏡下活檢11例,病理報告為子宮癌肉瘤、其他子宮惡性腫瘤、子宮內(nèi)膜不典型增生分別占36.4%、45.5%、9.1%。術(shù)前診斷性刮宮與宮腔鏡下活檢兩種方法診斷子宮癌肉瘤的符合率比較差異無統(tǒng)計學(xué)意義。(2)無附件轉(zhuǎn)移者,術(shù)前血清CA125升高者占23.5%,有附件轉(zhuǎn)移者術(shù)前CA125升高者占75.0%,兩者比較差異有統(tǒng)計學(xué)意義,術(shù)前血清CA125升高與附件轉(zhuǎn)移有關(guān)(P=0.002)。結(jié)論診斷性刮宮、宮腔鏡下活檢診斷子宮癌肉瘤的準(zhǔn)確性相近;術(shù)前組織學(xué)報告大多能診斷為惡性腫瘤,但組織學(xué)分類誤診率高。術(shù)前血清CA125升高患者,應(yīng)警惕附件轉(zhuǎn)移的可能性。
[Abstract]:Objective to investigate the value of hysteroscopic biopsy, diagnostic curettage two preoperative pathological examination method combined with serum CA125 in the diagnosis of uterine cancer sarcoma. Methods from January 2000 to January 2012 in Guangdong Province, endometrial cancer prevention project data is relatively complete, with 106 patients with pathologically confirmed uterine carcinosarcoma, on its preoperative examination method retrospective analysis. Results (1) preoperative diagnostic curettage in 74 cases, pathological report for uterine carcinosarcoma, other malignant tumor of uterus, endometrial atypical hyperplasia accounted for 35.1%, 52.7%, 9.5%; hysteroscopic biopsy 11 cases, diagnosed as uterine carcinosarcoma, other malignant tumor of uterus, endometrial atypical hyperplasia accounted for 36.4%, 45.5%, 9.1%. in the preoperative diagnosis rate had no significant difference with curettage under hysteroscopy and biopsy of two kinds of methods in the diagnosis of uterine carcinosarcoma. (2) no attachments , preoperative serum level of CA125 was 23.5%, with attachments metastasis elevated preoperative CA125 was 75%, the difference was statistically significant, preoperative serum CA125 associated with ovarian metastasis (P=0.002). Diagnostic curettage and similar accuracy of biopsy in diagnosis of uterine carcinosarcoma under hysteroscopy; preoperative histologic report most of the diagnosis of malignant tumors, but the histological classification of high misdiagnosis rate. The preoperative serum level of CA125 patients should be alert to the possibility of the attachments.
