網(wǎng)絡(luò)質(zhì)控陰道鏡在宮頸癌機(jī)會(huì)篩查中的臨床價(jià)值

【摘要】:目的:探討具有網(wǎng)絡(luò)質(zhì)控的陰道鏡在宮頸癌機(jī)會(huì)篩查中的臨床價(jià)值。方法:回顧分析2013年11月到2014年11月到重慶醫(yī)科大學(xué)附屬第二醫(yī)院就診的831例患者的具有網(wǎng)絡(luò)質(zhì)控的陰道鏡、宮頸液基細(xì)胞(TCT)、人乳頭瘤病毒(HPV)及宮頸活檢病理檢查結(jié)果;以病理檢查為金標(biāo)準(zhǔn),比較具有網(wǎng)絡(luò)質(zhì)控的陰道鏡、TCT、HPV單獨(dú)以及聯(lián)合對(duì)宮頸病變篩查的靈敏度、特異度、陽性預(yù)測(cè)值、陰性預(yù)測(cè)值及ROC曲線下面積。結(jié)果:具有網(wǎng)絡(luò)質(zhì)控的陰道鏡對(duì)≥CINⅡ病變檢出的特異度、陽性預(yù)測(cè)值及ROC曲線下面積分別為96.30%、85.63%、0.874,明顯高于TCT、HPV單獨(dú)或聯(lián)合檢測(cè)(P0.05);具有網(wǎng)絡(luò)質(zhì)控的陰道鏡聯(lián)合HPV檢測(cè)的靈敏度、陰性預(yù)測(cè)值分別為98.28%及99.28%,明顯高于其他方法單一或聯(lián)合檢測(cè)(P0.05)。Ⅰ型宮頸轉(zhuǎn)化區(qū)下陰道鏡對(duì)≥CINⅡ病變檢出的靈敏度、特異度、陽性預(yù)測(cè)值、陰性預(yù)測(cè)值及ROC曲線下面積分別為92.71%、98.67%、94.68%、98.15%、0.957,明顯高于Ⅱ型及Ⅲ型宮頸轉(zhuǎn)化區(qū)(P0.05)。結(jié)論:具有網(wǎng)絡(luò)質(zhì)控的陰道鏡可用于宮頸癌機(jī)會(huì)篩查;宮頸不同轉(zhuǎn)化區(qū)類型對(duì)陰道鏡診斷結(jié)果有一定影響,對(duì)Ⅱ型及Ⅲ型宮頸轉(zhuǎn)化區(qū)需聯(lián)合其他篩查手段。
