【摘要】：目的探討治療性宮頸環(huán)扎術(shù)的臨床效果及其影響因素。方法收集2008年1月至2015年7月于北京大學(xué)第三醫(yī)院接受治療性宮頸環(huán)扎術(shù)與未接受治療性環(huán)扎的宮頸功能不全的單胎妊娠患者132例,按孕期超聲或體格檢查發(fā)現(xiàn)的宮頸長度及宮頸內(nèi)外口擴張程度進行分組。宮頸長≤2.5 cm伴宮頸內(nèi)口呈V形或U形擴張,宮頸外口閉合者58例接受環(huán)扎(A組),未接受環(huán)扎23例(a組);宮頸內(nèi)外口均開大,同時伴或不伴胎囊突出者39例接受環(huán)扎(B組),未接受環(huán)扎12例(b組)。B組再按宮頸外口開大程度分:B1組為1 cm≤宮頸外口開大≤3 cm(27例),B2組為3 cm宮頸外口開大≤5 cm(9例),B3組為宮頸外口開大5 cm(3例)。分析比較臨床效果及與環(huán)扎效果有關(guān)的影響因素。結(jié)果 A組分娩孕周、妊娠延長天數(shù)、新生兒出生體重以及早期新生兒存活率均大于a組(P0.05);34周后及37周以后的分娩率明顯增加(P0.05)。B組妊娠延長天數(shù)、分娩孕周、新生兒出生體重以及28周、32周、34周的分娩率均明顯大于b組(P0.05);B組37周的分娩率大于b組(22.5%vs.0)。宮頸外口開大程度與環(huán)扎效果分析顯示:環(huán)扎治療B1組的妊娠延長天數(shù)及分娩孕周大于B2組,28周、32周及34周的分娩率明顯增加(P0.05)。環(huán)扎A組保胎天數(shù)、分娩孕周、34周以后及37周后分娩率均大于環(huán)扎B1組及B2組(P0.05)。B3組3例中僅有1例早期新生兒存活。多因素線性回歸分析顯示:胎囊突出情況、未足月胎膜早破、宮內(nèi)感染與術(shù)后保胎天數(shù)呈負相關(guān)(β分別為-20.869、-17.431、-14.963)。不同環(huán)扎孕周對分娩孕周、新生兒出生體重、早期新生兒存活率影響無統(tǒng)計學(xué)意義(P0.05)。結(jié)論治療性宮頸環(huán)扎術(shù)是妊娠期宮頸明顯變化的宮頸功能不全抗早產(chǎn)的有效補救方法;宮內(nèi)感染、未足月胎膜早破以及胎囊突出情況是環(huán)扎效果的影響因素,而宮頸外口開大5 cm的環(huán)扎效果受多因素影響;手術(shù)孕周不需局限在24周內(nèi),個體化處理為宜,手術(shù)效果不受手術(shù)孕周影響。
[Abstract]:Objective to investigate the clinical effect and influencing factors of therapeutic cervix cervix ligation. Methods from January 2008 to July 2015, the therapeutic and untreated cervix cervix were performed in the third Hospital of Peking University. 132 cases of single pregnancy with cervical dysfunction. According to the length of the cervix and the degree of dilatation of the cervical orifice found by ultrasound or physical examination during pregnancy, the cervix was less than 2.5 cm long and the cervical internal orifice was V-shaped or U-shaped.
