盆底肌肉鍛煉及鍛煉時機(jī)對產(chǎn)后盆底功能的影響研究

【摘要】:目的探討盆底肌肉鍛煉(PFMT)及鍛煉時機(jī)對產(chǎn)后盆底功能的影響,為女性產(chǎn)后選擇合適鍛煉時機(jī)提供依據(jù)。方法選取2014年2—9月于鄭州大學(xué)第二附屬醫(yī)院足月陰道分娩初產(chǎn)婦354例為研究對象,按病區(qū)將其隨機(jī)分為對照組(121例)、產(chǎn)后49 d組(114例)和產(chǎn)后7 d組(119例)。對照組僅進(jìn)行常規(guī)產(chǎn)后健康教育,產(chǎn)后7 d組與產(chǎn)后49 d組在此基礎(chǔ)上,分別于產(chǎn)后第7、49天開始采用Kegel訓(xùn)練進(jìn)行為期6周的PFMT,并書寫家庭訓(xùn)練日志。分別于產(chǎn)后49 d、3個月、6個月、12個月隨訪盆底肌力、盆腔臟器脫垂及壓力性尿失禁(SUI)發(fā)生情況。結(jié)果隨訪至產(chǎn)后12個月時,對照組、產(chǎn)后49 d組、產(chǎn)后7 d組分別有16、17、20例失訪。3組受試者產(chǎn)后49 d、3個月、6個月、12個月盆底Ⅰ、Ⅱ類肌肌力比較,差異均有統(tǒng)計學(xué)意義(P0.05);其中,產(chǎn)后49 d組產(chǎn)后3個月、6個月、12個月Ⅰ、Ⅱ類肌肌力優(yōu)于對照組(P0.016),產(chǎn)后7 d組產(chǎn)后49 d、3個月、6個月、12個月盆底Ⅰ、Ⅱ類肌肌力優(yōu)于產(chǎn)后49 d組與對照組(P0.016)。3組受試者產(chǎn)后49 d、3個月、6個月、12個月子宮脫垂、后壁脫垂發(fā)生率及前壁脫垂等級比較,差異均無統(tǒng)計學(xué)意義(P0.05)。3組受試者產(chǎn)后49 d、3個月、6個月、12個月SUI發(fā)生率比較,差異均有統(tǒng)計學(xué)意義(P0.05);其中產(chǎn)后7 d組產(chǎn)后49 d、3個月、6個月、12個月SUI發(fā)生率低于產(chǎn)后49 d組與對照組(P0.016)。結(jié)論產(chǎn)后早期進(jìn)行PFMT能有效提高盆底肌力,降低SUI發(fā)生風(fēng)險。
[Abstract]:Objective to investigate the effects of PFMTs on pelvic floor function in postpartum patients. Methods 354 puerperas with term vaginal delivery in the second affiliated Hospital of Zhengzhou University from 2014 to September were selected as the study objects. They were randomly divided into control group (121 cases), postpartum 49 day group (114 cases) and postpartum 7 day group (119 cases). The control group was only given routine postpartum health education. On the basis of this, the group of 7 days postpartum and the group of 49 days of postpartum began to use Kegel training for 6 weeks on the 749th day of postpartum. Family training log was written. Pelvic floor muscle strength was followed up at 49 days, 3 months, 6 months and 12 months postpartum, respectively.
