【摘要】：1目的近年來妊娠期高血壓疾病(HDCP)患者尤其是子癇前期孕婦增多,成為我國孕產(chǎn)婦和圍產(chǎn)兒發(fā)病及死亡的主要原因之一。為了尋找能靈敏的反映妊娠期高血壓疾病病理變化的有效指標(biāo),研究血管內(nèi)皮生長因子(VEGF)和5-羥色胺(5-HT)在妊娠期高血壓疾病患者與正常妊娠婦女外周血中的表達(dá)情況,探討其與妊娠高血壓疾病發(fā)病的關(guān)系,為早期預(yù)防HDCP的發(fā)生及臨床診斷和治療提供理論依據(jù)。方法1.選取滕州市中心人民醫(yī)院2012年11月-2013年10月在產(chǎn)科住院的妊娠期高血壓疾病患者50例為實(shí)驗(yàn)組(年齡23-35歲),其中妊娠期高血壓組30例,子癇前期組20例。選取同期在我院住院的正常妊娠婦女52例為對(duì)照組(年齡24-37歲),選擇的所有病例均排除內(nèi)外科合并癥及其他病理產(chǎn)科疾病如:前置胎盤、胎盤早剝、胎膜早破等。2.應(yīng)用雙抗體酶聯(lián)免疫吸附法(ELISA)檢測(cè)對(duì)照組與實(shí)驗(yàn)組外周血中VEGF和應(yīng)用高效液相色譜-電化學(xué)檢測(cè)(HPLC-ECD)法檢測(cè)對(duì)照組與實(shí)驗(yàn)組外周血中5-HT的表達(dá)情況。3.采用SPSS統(tǒng)計(jì)軟件進(jìn)行方差分析,兩組間均數(shù)比較采用t檢驗(yàn),血管內(nèi)皮生長因子(VEGF)與5-羥色胺(5-HT)水平的相關(guān)性采用Pearson相關(guān)分析,以a=0.05為檢驗(yàn)水準(zhǔn),P0.05為差異有統(tǒng)計(jì)學(xué)意義。結(jié)果1.妊娠期高血壓疾病孕婦血清VEGF水平明顯低于正常孕婦,P0.05差異有統(tǒng)計(jì)學(xué)意義。2.妊娠期高血壓組血清VEGF水平較子癇前期組高,P0.05差異有統(tǒng)計(jì)學(xué)意義。3.隨妊娠期高血壓疾病血壓的升高VEGF水平呈逐漸下降趨勢(shì),P0.05差異有統(tǒng)計(jì)學(xué)意義。4.妊娠期高血壓疾病孕婦血漿中5-HT較正常孕婦明顯升高,P0.05差異有統(tǒng)計(jì)學(xué)意義。5.子癇前期組血清中5-HT水平較妊娠期高血壓組升高,P0.05差異有統(tǒng)計(jì)學(xué)意義。6.妊娠期高血壓疾病組血VEGF與5-HT水平呈負(fù)相關(guān),而在正常妊娠組中兩者無明顯相關(guān)性。結(jié)論1.VEGF和5-HT與妊娠期高血壓疾病特別是子癇前期的發(fā)生及病情進(jìn)展關(guān)系密切。2.VEGF在維持正常妊娠和胎兒生長發(fā)育過程中起積極作用。3.5-HT與妊娠期高血壓疾病有一定的相關(guān)性。4.妊娠后母體血液中VEGF水平顯著下降可能是妊娠期高血壓疾病發(fā)病機(jī)理中的一個(gè)重要因素。5.VEGF和5-HT的水平變化反映妊娠期高血壓疾病病情的動(dòng)態(tài)變化,可作為妊娠期高血壓疾病早期預(yù)測(cè)、臨床診斷和治療的參考依據(jù)。
[Abstract]:Objective in recent years, the number of patients with hypertension complicating pregnancy (HDCPD), especially preeclampsia, has increased. It has become one of the main causes of morbidity and mortality of pregnant women and perinatal infants in China. In order to find an effective index to reflect the pathological changes of hypertensive disorder complicating pregnancy. To study the expression of vascular endothelial growth factor (VEGF) and serotonin (5-HT) in peripheral blood of hypertensive disorder complicating pregnancy and normal pregnant women. Objective: to investigate the relationship between pregnancy hypertension and the incidence of pregnancy hypertension. To provide theoretical basis for early prevention, clinical diagnosis and treatment of HDCP. 1. Select the pregnant women hospitalized in obstetrics department of Tengzhou central people's hospital from November 2012 to October 2013. The experimental group consisted of 50 patients with compression disease (. Ages 23 to 35 years). There were 30 cases of hypertensive complicating pregnancy group and 20 cases of preeclampsia group. 52 normal pregnant women in our hospital were selected as control group (aged 24-37 years). All the selected cases were excluded from internal and surgical complications and other pathological obstetrical diseases such as placenta previa and placental abruption. Detection of VEGF in peripheral blood of control group and experimental group by double antibody enzyme-linked immunosorbent assay (Elisa) and high performance liquid chromatography-electrochemical detection (HPLC-ECD). Methods to detect the expression of 5-HT in peripheral blood of control group and experimental group by using SPSS software. The correlation between vascular endothelial growth factor (VEGF) and serotonin (5-HT) was analyzed by Pearson correlation analysis. Results 1. The serum VEGF level of pregnant women with hypertension complicating diseases was significantly lower than that of normal pregnant women. 2. The serum VEGF level of hypertensive complicating pregnancy group was higher than that of preeclampsia group. P0.05 difference was statistically significant. 3. With the increase of blood pressure of gestational hypertension disorder, the level of VEGF decreased gradually. P0.05 difference was statistically significant. 4. Gestational hypertension complicating hypertension in maternal plasma 5-HT significantly higher than normal pregnant women. The serum 5-HT level in preeclampsia group was higher than that in gestational hypertension group. P0.05 the difference was statistically significant. 6. The serum VEGF and 5-HT levels were negatively correlated in hypertensive disorder complicating pregnancy group. Conclusions 1. VEGF and 5-HT are closely related to the occurrence and progression of hypertensive disorder complicating pregnancy, especially preeclampsia. 3.5-HT plays a positive role in pregnancy and fetal development. 3.5-HT has a certain correlation with hypertensive disorder complicating pregnancy .4.After pregnancy, the level of VEGF in maternal blood decreased significantly, which may be the reason of hypertension complicating pregnancy. An important factor in the pathogenesis of the disease. 5. The changes of VEGF and 5-HT levels reflect the dynamic changes of hypertensive disorder complicating pregnancy. It can be used as a reference for early prediction, clinical diagnosis and treatment of hypertensive disorder complicating pregnancy.
