癲癇患兒海馬急性損傷的MRI表現(xiàn)

【摘要】:目的研究癲癇患兒海馬急性損傷的MRI表現(xiàn),探討其發(fā)病原因及轉(zhuǎn)歸,提高對(duì)海馬急性損傷結(jié)構(gòu)改變的認(rèn)識(shí)。資料與方法回顧性分析經(jīng)頭顱MRI檢查擴(kuò)散加權(quán)成像(DWI)海馬表現(xiàn)為高信號(hào)的8例癲癇患兒的臨床及影像學(xué)資料,測(cè)量雙側(cè)海馬體積及表觀擴(kuò)散系數(shù)(ADC)值,分析海馬體積、T2WI信號(hào)變化及ADC值改變。結(jié)果 8例臨床診斷為癲癇,其中7例有反復(fù)癲癇發(fā)作史。MRI檢查距癲癇發(fā)作時(shí)間1~5 d。8例均為單側(cè)海馬體積增大,其中左側(cè)6例,右側(cè)2例,患側(cè)海馬體積為(2.096±0.570)cm3,顯著大于健側(cè)(1.768±0.447)cm3(t=-4.763,P0.01)。T2WI及FLAIR信號(hào)增高;DWI顯示高信號(hào),增高約25.52%;紓(cè)ADC值為(0.763±0.049)×10-3 mm2/s,較健側(cè)(0.859±0.054)×10-3 mm2/s降低(t=9.598,P0.01)。3例此次發(fā)作前曾由于熱性驚厥或抽搐發(fā)作行MRI檢查,雙側(cè)海馬未見(jiàn)異常。3例3~5個(gè)月后復(fù)查,患側(cè)海馬體積縮小;T2WI及FLAIR信號(hào)仍較高;ADC值增高15.6%,表現(xiàn)為海馬硬化。結(jié)論癲癇可以引起海馬急性損傷,表現(xiàn)為海馬腫大,T2WI信號(hào)增加,ADC值下降;海馬急性損傷發(fā)展為海馬硬化。
