基于自動(dòng)分割技術(shù)聯(lián)合基于體素的形態(tài)學(xué)觀察帕金森病腦灰質(zhì)結(jié)構(gòu)

【摘要】:目的探討基于自動(dòng)分割技術(shù)聯(lián)合基于體素的形態(tài)學(xué)(VBM)觀察帕金森病(PD)患者全腦灰質(zhì)異常區(qū)域及分布特征的應(yīng)用價(jià)值。方法基于自動(dòng)分割技術(shù),應(yīng)用FIRST工具對(duì)29例PD患者(PD組)及30名健康人(對(duì)照組)的T1圖像皮層下灰質(zhì)結(jié)構(gòu)精確分割,對(duì)比兩組各灰質(zhì)結(jié)構(gòu)體積。并應(yīng)用VBM方法對(duì)兩組腦灰質(zhì)圖像進(jìn)行比較。結(jié)果兩組右側(cè)殼核皮層下灰質(zhì)體積差異有統(tǒng)計(jì)學(xué)意義(t=10.201,P0.05)。與對(duì)照組比較,PD組腦灰質(zhì)體積(右側(cè)初級(jí)運(yùn)動(dòng)皮層,雙側(cè)額葉、邊緣葉、部分左側(cè)小腦后葉、右側(cè)小腦前葉、右側(cè)小腦后葉、右側(cè)顳葉、頂葉、殼核及左側(cè)枕葉)廣泛減少,部分左側(cè)小腦后葉體積增加,兩側(cè)半球腦體積缺失不對(duì)稱(右側(cè)大于左側(cè))。結(jié)論通過(guò)FIRST工具可精確分割并直接計(jì)算皮層下灰質(zhì)結(jié)構(gòu)體積,應(yīng)用VBM技術(shù)可定量分析腦結(jié)構(gòu)形態(tài)學(xué)異常;二者結(jié)合可較全面地表現(xiàn)PD腦灰質(zhì)體積廣泛減少的形態(tài)學(xué)特點(diǎn)。
