小腿平衡穩(wěn)態(tài)自由進(jìn)動(dòng)序列非增強(qiáng)MR血管成像與增強(qiáng)MR血管成像的對(duì)比

【摘要】:目的通過與增強(qiáng)磁共振血管成像(CE MRA)圖像比較,探討3.0T MR小腿血流敏感散相(FSD)MRA的可行性。方法收集20例可疑下肢動(dòng)脈疾病患者,行小腿FSD MRA及下肢CE MRA,比較其圖像質(zhì)量;并以CE MRA為診斷標(biāo)準(zhǔn),判斷FSD MRA的診斷效能。結(jié)果 FSD MRA有效FOV約為(275.93±29.76)mm,圖像質(zhì)量評(píng)價(jià)觀察者間一致性良好(Kappa=0.81,P0.01);谘芄(jié)段,FSD MRA圖像質(zhì)量評(píng)分[(3.66±0.81)分]略優(yōu)于CE MRA[(3.49±0.87)分,P=0.05],可用于診斷的血管節(jié)段數(shù)間兩者差異無統(tǒng)計(jì)學(xué)意義[FSD MRA:97.65%(166/170),CEMRA:98.24%(167/170);P=0.32];诨颊,FSD MRA及CE MRA的圖像質(zhì)量差異無統(tǒng)計(jì)學(xué)意義[(3.25±1.06)分vs(2.94±1.06)分,P=0.40]。血管狹窄程度評(píng)價(jià)觀察者間一致性良好(Kappa=0.81,P0.01);谘芄(jié)段,FSD MRA與CE MRA對(duì)于血管狹窄程度評(píng)價(jià)的符合率為84.34%;FSD MRA檢測(cè)血管狹窄的敏感度、特異度、陽性預(yù)測(cè)值和陰性預(yù)測(cè)值分別為96.41%、93.04%、84.06%和98.53%。基于患者,FSD MRA與CE MRA評(píng)價(jià)血管狹窄程度差異無統(tǒng)計(jì)學(xué)意義[(3.27±1.10)分vs(3.27±1.10)分,P=1.00]。結(jié)論 FSD應(yīng)用于3.0T MRA可為下肢動(dòng)脈病變提供一項(xiàng)可靠的非增強(qiáng)MRA檢查方法。
