【摘要】：目的:探討乳腺腫瘤超聲造影無灌注區(qū)的臨床意義及其發(fā)生的病理基礎(chǔ)。方法:回顧性分析120例乳腺腫瘤的超聲造影動態(tài)圖像,仔細(xì)觀察這些圖像中是否出現(xiàn)無灌注區(qū),并進(jìn)一步分析無灌注區(qū)的面積和部位等相應(yīng)參數(shù),研究其與腫塊大小及病理結(jié)果中出現(xiàn)的特征性表現(xiàn)(壞死、出血、鈣化和黏液樣變)的相關(guān)性。結(jié)果:120例乳腺腫瘤超聲造影圖像中,93例出現(xiàn)無灌注區(qū),在良惡性之間存在統(tǒng)計學(xué)差異(P=0.000,r=0.442),出現(xiàn)無灌注區(qū)與壞死之間有統(tǒng)計學(xué)差異(P=0.000,r=0.336)。但是無灌注區(qū)面積及部位與腫瘤大小,腫瘤良惡性及各種病理改變之間均無統(tǒng)計學(xué)意義(P0.05)。結(jié)論:超聲造影無灌注區(qū)的出現(xiàn)有助于乳腺癌的診斷、壞死等病理變化為其提供病理基礎(chǔ)。有關(guān)無灌注區(qū)發(fā)生發(fā)展深入的病理基礎(chǔ)尚有待于進(jìn)一步的大樣本研究。
