【摘要】：目的應(yīng)用超聲彈性成像技術(shù)評價(jià)下肢深靜脈血栓硬度,探討血栓硬度與短期溶栓效果間的相關(guān)性及應(yīng)變率比值法在預(yù)測溶栓效果中的應(yīng)用價(jià)值。方法對63例急性下肢深靜脈血栓患者行超聲彈性成像(UE)檢查并測定應(yīng)變率比值(SR);經(jīng)過10d的抗凝、溶栓治療,根據(jù)治療效果不同,將其分為治愈組、治療有效組、治療無效組,比較各組間血栓SR的差異;將血栓SR與治療是否有效對照,構(gòu)建受試者工作特征曲線(ROC)并確定溶栓是否有效的SR臨界值。結(jié)果 (1)血栓硬度與溶栓效果具有一定相關(guān)性。治愈組及治療有效組的血栓SR明顯小于治療無效組。治愈組與治療有效組間血栓SR無明顯差異。(2)血栓SR可以預(yù)測溶栓治療效果。以SR1.36預(yù)測治療有效,以SR≥1.36預(yù)測治療無效,其靈敏度、特異度、準(zhǔn)確度分別為88.9%、94.4%、93.7%。結(jié)論血栓硬度與溶栓效果具有一定相關(guān)性,超聲彈性成像應(yīng)變率比值法可以用來評價(jià)血栓硬度并預(yù)測治療效果。
[Abstract]:Objective to evaluate the hardness of deep venous thrombosis of lower extremity by ultrasonic elastic imaging, to explore the correlation between thrombus hardness and short-term thrombolysis effect, and to explore the value of strain rate ratio method in predicting the effect of deep venous thrombosis. Methods 63 patients with acute deep venous thrombosis of lower extremity were examined by ultrasonic elastic imaging (UE) and the strain rate ratio (SR);) was measured. After 10 days of anticoagulant and thrombolysis therapy, according to the different therapeutic effects, it was divided into cure group, effective group and ineffective group, and the difference of thrombus SR among groups was compared. Compared with whether thrombus SR was effective or not, the working characteristic curve (ROC) of the subjects was constructed and the critical value of SR was determined whether the thrombus was effective or not. Results (1) there was a certain correlation between thrombus hardness and thrombolysis effect. The thrombus SR in the healing group and the effective group was significantly lower than that in the ineffective group. There was no significant difference in thrombus SR between cured group and effective group. (2) thrombus SR could predict the effect of thrombus therapy.
