【摘要】：目的:探討核磁共振與PET-CT診斷肝臟泡狀棘球蚴病(HAE)的應(yīng)用價(jià)值。方法:回顧性分析經(jīng)手術(shù)病理證實(shí)為HAE且同時(shí)行MR常規(guī)掃描和PET-CT檢查的14例患者資料。14例HAE患者共檢出30個(gè)病灶,按照Kodama的MRI分型分為5種類(lèi)型,分別觀察MRI和PET-CT表現(xiàn),并根據(jù)MRI和PET-CT結(jié)果對(duì)HAE病灶的生物學(xué)活性進(jìn)行定性評(píng)估。對(duì)MRI和PET-CT結(jié)果進(jìn)行一致性分析,計(jì)算Kappa值。結(jié)果:(1)MRI表現(xiàn):30個(gè)HAE病灶最大徑平均值約為14.59cm,1型1個(gè)病灶,2型9個(gè)病灶,3型10個(gè)病灶,4型10個(gè)病灶。(2)PET-CT表現(xiàn):30個(gè)HAE病灶中,16個(gè)病灶顯示18F脫氧葡萄糖(18F-FDG)在病灶邊緣呈不同程度攝取,其中1、2、3型(20個(gè)病灶)共有15個(gè)病灶顯示18F脫氧葡萄糖(18F-FDG)呈不同程度攝取,4型10個(gè)病灶中有1個(gè)病灶顯示18F脫氧葡萄糖(18F-FDG)呈高攝取。14例共30個(gè)病灶通過(guò)MRI和PET-CT對(duì)病灶生物學(xué)活性評(píng)估的一致性較好(Kappa=0.59,P0.01)。結(jié)論:基于MRI圖像有無(wú)小囊泡顯示可以反映HAE是否存在生物學(xué)活性。
[Abstract]:Objective: to evaluate the value of magnetic resonance imaging (MRI) and PET-CT in the diagnosis of hepatic alveolar echinococcosis (HAE). Methods: the data of 14 patients with HAE confirmed by surgery and pathology were retrospectively analyzed. 14 patients with HAE were divided into 5 types according to the MRI classification of Kodama, 30 lesions were detected by routine MR scanning and PET-CT examination, the patients were divided into 5 types according to the MRI classification of Kodama, and 30 lesions were detected in 14 patients with HAE. The features of MRI and PET-CT were observed, and the biological activity of HAE lesions was evaluated qualitatively according to the results of MRI and PET-CT. The consistency of MRI and PET-CT results was analyzed and the Kappa value was calculated. Results: (1) MRI findings: the maximum diameter of 30 HAE lesions was 14.59 cm, 1 focus of type 1, 9 lesions of type 2, 10 lesions of type 3, 10 lesions of type 4. (2) PET-CT findings: of the 30 lesions of HAE, there were 1 focus of type 1, 9 lesions of type 2, 10 lesions of type 4. Sixteen lesions showed 18F-deoxyglucose (18F-FDG) uptake in different degrees at the edge of the lesion, of which 15 lesions showed different uptake of 18F-deoxyglucose (18F-FDG) in 1,2,3-type (20 lesions). One of the 10 lesions of type 4 showed high uptake of 18F-deoxyglucose (18F-FDG), and a total of 30 lesions in 14 cases had a good consistency (Kappa=0.59,P0.01) in evaluating the biological activity of the lesions by MRI and PET-CT. Conclusion: the presence or absence of vesicles in MRI images can reflect the biological activity of HAE.
