超聲“螢火蟲”成像技術(shù)在乳腺腫塊微鈣化檢出中的價(jià)值及臨床意義

【摘要】:目的探討超聲"螢火蟲"成像技術(shù)對(duì)乳腺腫塊微鈣化的檢出能力及其在乳腺腫瘤診斷中的價(jià)值。方法乳腺腫塊患者153例(172個(gè)病灶)。行鉬靶X線、常規(guī)超聲及超聲"螢火蟲"成像技術(shù)檢查,觀察腫塊微鈣化情況。以鉬靶X線檢查結(jié)果為參照,評(píng)價(jià)超聲"螢火蟲"成像技術(shù)對(duì)微鈣化的檢出情況;以病理檢查結(jié)果為參照,評(píng)價(jià)超聲診斷乳腺惡性腫瘤的準(zhǔn)確度。結(jié)果 (1)超聲"螢火蟲"成像技術(shù)檢出乳腺腫塊內(nèi)微鈣化的特異度97.8%、靈敏度98.8%和準(zhǔn)確度98.3%均明顯高于常規(guī)超聲的83.5%、49.4%和67.4%,差異具有統(tǒng)計(jì)學(xué)意義(P0.05);(2)不同形態(tài)的微鈣化灶,超聲"螢火蟲"成像均表現(xiàn)為點(diǎn)狀強(qiáng)回聲;鉬靶X線檢查成簇狀分布和區(qū)域樣分布的微鈣化,超聲"螢火蟲"成像表現(xiàn)為簇狀分布,散在分布的微鈣化也表現(xiàn)為散在分布;(3)ROC分析結(jié)果顯示:超聲"螢火蟲"成像技術(shù)診斷乳腺惡性腫瘤的準(zhǔn)確度明顯高于常規(guī)超聲,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論超聲"螢火蟲"成像技術(shù)可提高乳腺腫塊微鈣化的檢出率,顯示微鈣化分布特征,從而提高超聲診斷乳腺惡性腫瘤的準(zhǔn)確度。
[Abstract]:Objective to investigate the detection of microcalcification in breast masses by ultrasound firefly imaging and its value in the diagnosis of breast tumors. Methods 153 patients (172 lesions) with breast mass were included. Mammography, conventional ultrasound and firefly imaging were performed to observe the microcalcification of the tumor. The detection of microcalcification by ultrasonic "firefly" imaging technique and the accuracy of ultrasonic diagnosis of breast malignant tumor were evaluated by using the results of mammography as the reference. Results (1) the specificity of ultrasound "firefly" imaging in detecting microcalcification in breast masses was 97.8. the sensitivity and accuracy were 98.8% and 98.3%, respectively, which were significantly higher than those of conventional ultrasound (83.5% and 67.4%). The difference was statistically significant (P0.05). (2) the ultrasound "firefly" imaging of the microcalcified foci with different morphology showed the dot strong echo; The microcalcification of cluster distribution and regional distribution was detected by molybdenum target X-ray. The ultrasonic "firefly" imaging showed cluster distribution, and the scattered microcalcification was also distributed. (3) the results of ROC analysis showed that the accuracy of ultrasound "firefly" imaging in the diagnosis of breast malignant tumors was significantly higher than that of conventional ultrasound, and the difference was statistically significant (P0.05). Conclusion Ultrasound "firefly" imaging technique can improve the detection rate of microcalcification in breast masses and display the distribution of microcalcification, thus improving the accuracy of ultrasound in the diagnosis of breast malignant tumors.
