【摘要】：目的探討MRI對顱內(nèi)不典型表皮樣囊腫的診斷價值。方法搜集26例經(jīng)手術病理證實的不典型表皮樣囊腫患者MRI T1WI、T2WI、擴散加權成像(DWI)及增強檢查資料,分析其部位、形態(tài)及信號特點。結果 11例表皮樣囊腫位于腦室內(nèi),12例位于腦實質(zhì)內(nèi),2例位于硬膜外,1例位于大腦縱裂前部。呈類圓形、不規(guī)則或分葉狀,周圍均無水腫。腦室內(nèi)、大腦縱裂腫瘤信號呈均勻或不均勻長T1、長T2信號;腦實質(zhì)內(nèi)、硬膜外腫瘤信號復雜多樣,T1WI、T2WI呈混雜信號,可見T1WI高信號,或T2WI低信號。DWI均呈明顯高信號。增強掃描4例囊壁強化。結論 DWI結合T1WI、T2WI及增強掃描對顱內(nèi)不典型表皮樣囊腫的診斷及鑒別診斷有重要價值。
[Abstract]:Objective to evaluate the diagnostic value of MRI in intracranial atypical epidermoid cysts. Methods 26 cases of atypical epidermoid cysts confirmed by operation and pathology were collected. The data of MRI T 1WI T 2WI, diffusion weighted imaging (DWI) and contrast enhancement were collected, and their location, morphology and signal characteristics were analyzed. Results 11 cases of epidermoid cysts were located in the ventricle, 12 cases in the cerebral parenchyma, 2 cases in the epidural area, and 1 case in the anterior part of the longitudinal fissure of the brain. It was round, irregular or lobulated, and there was no edema around it. In the ventricle, the signal intensity of the longitudinal fissure tumor was homogeneous or uneven with long T 1 and long T 2 signal. In the brain parenchyma, the signal intensity of epidural tumor was complex and varied. The signal intensity of T1WI T2WI was mixed, T1WI was high signal, or T2WI signal was low. DWI was obviously high signal intensity. Enhancement scanning was performed in 4 cases with cystic wall enhancement. Conclusion DWI combined with T _ 1WI T _ 2WI and contrast-enhanced scanning have important value in the diagnosis and differential diagnosis of intracranial atypical epidermoid cysts.
