慢性心衰患者超聲心功能指標(biāo)與血清Hcy的相關(guān)性

【摘要】:目的:研究血清同型半胱氨酸(Hcy)水平與慢性心力衰竭(CHF)患者超聲心功能指標(biāo)的相關(guān)性。方法:研究對(duì)象選取2014年4月至2015年6月間收治的CHF患者88例(心衰組),同期選擇進(jìn)行體檢的健康者65例作為對(duì)照組,采用彩色多普勒超聲對(duì)兩組患者的左室射血分?jǐn)?shù)(LVEF)和左室舒張末期內(nèi)徑(LVEDd)等心功能指標(biāo)進(jìn)行測(cè)定,同時(shí)測(cè)定血清Hcy和N端B型腦鈉肽前體(NT-pro BNP)水平,比較兩組和不同心功能分級(jí)患者的血清Hcy和NT-pro BNP水平、LVEF及LVEDd差異,分析血清Hcy和NT-pro BNP水平與LVEF、LVEDd之間的相關(guān)性,分析血清Hcy水平對(duì)心血管事件影響。結(jié)果:CHF組的Hcy、NTpro BNP水平及LVEDd均顯著高于對(duì)照組(P0.01),LVEF明顯低于對(duì)照組(t=31.78,P=0.00);不同心功能分級(jí)(II~IV級(jí))的Hcy、NT-pro BNP、LVEDd及LVEF比較均具有統(tǒng)計(jì)學(xué)差異(P0.01);Pearson相關(guān)性分析顯示,CHF患者的血清Hcy水平與LVEF呈負(fù)相關(guān)(r2=0.974,P0.01),與LVEDd呈正相關(guān)(r~2=0.896,P0.05),且血清Hcy水平越高,CHF患者的心血管事發(fā)生率越高。結(jié)論:CHF患者的血清Hcy和NTpro BNP水平較健康者明顯升高,且隨著心功能惡化而增加,同時(shí)與LVEF呈負(fù)相關(guān),與LVEDd正相關(guān),高Hcy水平還可增加發(fā)生心血管事件風(fēng)險(xiǎn),因此Hcy水平有望作為CHF病情變化的新的診斷指標(biāo)。
[Abstract]:Objective: to study the relationship between serum homocysteine (Hcy) level and echocardiographic cardiac function in patients with chronic heart failure (CHF). Methods: 88 patients with CHF (heart failure group) admitted from April 2014 to June 2015 and 65 healthy persons who underwent physical examination were selected as control group. Left ventricular ejection fraction (LVEF) and left ventricular end-diastolic diameter (LVEDd) were measured by color Doppler ultrasound. Serum Hcy and N-terminal B-type brain natriuretic peptide precursor (NT-pro BNP) were measured. The levels of serum Hcy and NT-pro BNP, LVEF and LVEDd were compared between the two groups and patients with different cardiac function grade. The correlation between serum Hcy and NT-pro BNP levels and LVEF,LVEDd was analyzed, and the influence of serum Hcy level on cardiovascular events was analyzed. Results: the levels of Hcy,NTpro BNP and LVEDd in CHF group were significantly higher than those in control group (P0.01), LVEF was significantly lower than that in control group (t 31.78). The difference of Hcy,NT-pro BNP,LVEDd and LVEF in different cardiac function grade (II~IV grade) was statistically significant (P0.01). Pearson correlation analysis showed that the level of serum Hcy in CHF patients was negatively correlated with LVEF (r2n0.974, P0.01), and positively correlated with LVEDd (P 0.05). The higher the level of serum Hcy, the higher the incidence of cardiovascular events in CHF patients. Conclusion: the serum Hcy and NTpro BNP levels in patients with CHF were significantly higher than those in healthy controls, and increased with the deterioration of cardiac function, and were negatively correlated with LVEF and positively correlated with LVEDd. High Hcy levels also increased the risk of cardiovascular events. Therefore, the level of Hcy is expected to be a new diagnostic index for the changes of CHF.
