【摘要】：目的探討剪切波彈性成像對(duì)橋本甲狀腺炎患者合并甲狀腺結(jié)節(jié)良惡性的鑒別診斷價(jià)值。方法收集經(jīng)穿刺活檢或術(shù)后病理證實(shí)的橋本甲狀腺炎患者54例,根據(jù)結(jié)節(jié)良惡性分為良性結(jié)節(jié)組和惡性結(jié)節(jié)組,良性結(jié)節(jié)組又分為假結(jié)節(jié)組和真結(jié)節(jié)組。測(cè)量并比較分析甲狀腺結(jié)節(jié)的楊氏模量值,繪制ROC曲線以評(píng)價(jià)其診斷效能。結(jié)果 54例橋本甲狀腺炎患者共檢出162個(gè)結(jié)節(jié),其中良性結(jié)節(jié)131個(gè)(真結(jié)節(jié)93個(gè),假結(jié)節(jié)38個(gè))、惡性結(jié)節(jié)31個(gè)。橋本甲狀腺炎合并良性結(jié)節(jié)、惡性結(jié)節(jié)、真結(jié)節(jié)及假結(jié)節(jié)的楊氏模量均值分別為(33.04±10.95)kPa、(66.15±19.17)kPa、(35.95±9.74)kPa、(30.13±11.87)kPa。良性結(jié)節(jié)與惡性結(jié)節(jié)楊氏模量值差異有統(tǒng)計(jì)學(xué)意義(t=12.91,P0.01);假結(jié)節(jié)組、真結(jié)節(jié)組與惡性結(jié)節(jié)組的楊氏模量值總體差異有統(tǒng)計(jì)學(xué)意義(F=36.20,P0.01),且惡性結(jié)節(jié)組楊氏模量值高于其他2組(P均0.01)。通過(guò)ROC曲線確定楊氏模量值的診斷臨界點(diǎn)為43.65 kPa,其診斷良惡性甲狀腺結(jié)節(jié)的敏感度為90.63%、特異度為81.25%、陽(yáng)性似然比為4.83、陰性似然比為0.12。結(jié)論剪切波彈性成像技術(shù)可以對(duì)橋本甲狀腺炎及其結(jié)節(jié)進(jìn)行定量分析,有助于結(jié)節(jié)性質(zhì)的判定。
[Abstract]:Objective to evaluate the value of shear wave elastography in differential diagnosis of benign and malignant thyroid nodules in patients with Hashimoto's thyroiditis. Methods 54 patients with Hashimoto's thyroiditis confirmed by puncture biopsy or postoperative pathology were divided into benign and malignant nodule group and benign nodule group. The benign nodule group was divided into pseudodular group and true nodule group. The Young's modulus of thyroid nodule was measured and analyzed, and the ROC curve was drawn to evaluate its diagnostic efficacy. Results 162 nodules were detected in 54 patients with Hashimoto's thyroiditis, including 131 benign nodules (93 true nodules, 38 pseudonodules) and 31 malignant nodules. The average Young's modulus of Hashimoto's thyroiditis with benign, malignant, true and pseudodular nodules was (33.04 鹵10.95) kPa, () 66.15 鹵19.17) kPa, (35.95 鹵9.74) kPa, (30.13 鹵11.87) kPa., respectively. There was significant difference in Young's modulus between benign and malignant nodules (t = 12.91, P0.01). There was significant difference in Young's modulus between pseudodular group, true nodule group and malignant nodule group (F _ (36.20) P _ (0.01), and the Young's modulus value in malignant nodule group was higher than that in other two groups (P < 0.01). The diagnostic critical point of Young's modulus was 43.65 kPa, by ROC curve. The sensitivity, specificity, positive likelihood ratio and negative likelihood ratio for benign and malignant thyroid nodules were 90.63, 81.25, 4.83 and 0.12, respectively. Conclusion Shear wave elastography can be used to quantitatively analyze Hashimoto's thyroiditis and its nodules, which is helpful to the determination of nodular properties.
