【摘要】：目的探討影響經(jīng)陰道超聲(TVS)診斷早期異位妊娠(EP)準(zhǔn)確率的因素。方法回顧性分析5600例停經(jīng)4~6周患者中205例疑似EP的聲像圖,并與腹腔鏡結(jié)果對照。結(jié)果 TVS診斷早期EP準(zhǔn)確率為68.86%(115/167);妊娠位置不確定(PUL)發(fā)生率38.54%(79/205);早期妊娠無明顯臨床癥狀比例為53.66%(110/205);未破裂型EP包塊中輸卵管環(huán)型居多(110/155,70.97%);年資短的醫(yī)師較年資長的醫(yī)師易發(fā)生誤診(P0.05)。結(jié)論影響TVS診斷早期EP的主要因素為PUL的存在、缺乏臨床癥狀、未破裂EP超聲圖像不典型及超聲醫(yī)師的經(jīng)驗和技能。
[Abstract]:Objective to investigate the factors influencing the accuracy of transvaginal ultrasound (TVS) in the diagnosis of early ectopic pregnancy (EP). Methods the sonogram of suspected EP in 5600 postmenopausal patients at 6 weeks postmenopause was retrospectively analyzed and compared with the results of laparoscopy. Results the accuracy rate of TVS in the diagnosis of early EP was 68.86% (115 / 167), the incidence of (PUL) was 38.54% (79 / 205), the rate of no obvious clinical symptoms in early pregnancy was 53.66% (110 / 205). Most of the unruptured EP masses were oviduct ring type (110 / 1550.97%), and the doctors with short seniority were more likely to misdiagnose than those with long service (P0.05). Conclusion the main factors affecting the early diagnosis of EP by TVS are the presence of PUL, the lack of clinical symptoms, the atypical ultrasound images of unruptured EP and the experience and skills of ultrasound physicians.
