磁共振擴散張量成像在評價兒童腦內(nèi)部分皮質(zhì)脊髓束發(fā)育中的應(yīng)用

【摘要】:目的應(yīng)用磁共振擴散成像技術(shù)探討兒童皮質(zhì)脊髓束年齡和性別差異及其變化規(guī)律。方法 90例無中樞神經(jīng)系統(tǒng)癥狀及體征且顱腦磁共振檢查正常的兒童(年齡5d~18歲)。按年齡分為5組:嬰兒組(組1,≤1歲),幼兒組(組2,1~3歲),學齡前組(組3,3~6歲),學齡組(組4,6~12歲),青春發(fā)育期組(組5,12~18歲)。每個年齡組內(nèi)再按性別分為男、女2組。各組兒童分別行頭部擴散張量成像,根據(jù)感興趣法選取皮質(zhì)脊髓束感興趣區(qū)并重建,測量重建的皮質(zhì)脊髓束的擴散張量參數(shù)并進行統(tǒng)計學分析。結(jié)果各年齡組的表觀擴散系數(shù)(ADC)值、分數(shù)各向異性(FA)值、纖維示蹤平均長度、體積以及示蹤的纖維束數(shù)量不完全相同(P0.01);組間的兩兩比較間發(fā)現(xiàn),1組與2組間ADC值、FA值及示蹤纖維平均長度差異具有統(tǒng)計學意義;2組與3組間FA值差異具有統(tǒng)計學意義;3組與4組間ADC值、FA值、示蹤纖維平均長度、纖維束數(shù)目及體積差異均具有統(tǒng)計學意義。ADC值與年齡間呈負相關(guān),余參數(shù)與年齡間呈正相關(guān)。結(jié)論兒童皮質(zhì)脊髓束發(fā)育具有階段性,且具有階段性特征;磁共振擴散張量技術(shù)可用于觀測兒童皮質(zhì)脊髓束,評價其發(fā)育狀況。
