不同睡眠狀態(tài)對(duì)卵泡期女性悲傷面孔識(shí)別影響的功能核磁共振成像研究

【摘要】:目的:探討不同睡眠狀態(tài)下,青年女性卵泡期悲傷面孔識(shí)別差異腦區(qū),為女性睡眠與情緒疾病之間的相關(guān)機(jī)制提供理論依據(jù)。 對(duì)象:招募大連醫(yī)科大68名健康女生志愿者為研究對(duì)象,根據(jù)匹茲堡睡眠量表、明尼蘇達(dá)多項(xiàng)人格問卷、特質(zhì)焦慮狀態(tài)問卷、生活事件、月經(jīng)周期曲線評(píng)估,篩查出16名健康女生進(jìn)入實(shí)驗(yàn)組,在下一次月經(jīng)的卵泡期,所有被試進(jìn)入腦功能實(shí)驗(yàn)。 方法:實(shí)驗(yàn)設(shè)計(jì)為自身對(duì)照研究,在三種狀態(tài)下觀察不同睡眠狀態(tài)下情感面孔識(shí)別差異。即:正常睡眠、24小時(shí)睡眠剝奪和4小時(shí)睡眠恢復(fù)三種狀態(tài)下觀看中性—悲傷情緒面孔圖片,同時(shí)行3.0T腦功能磁共振掃描收集激活的影像數(shù)據(jù)。使用SPM8軟件處理數(shù)據(jù),進(jìn)行分析。 結(jié)果:1.各量表分值比較:①主觀睡眠評(píng)分:單因素方差分析顯示差異顯著(P0.05);②狀態(tài)焦慮評(píng)分:正常睡眠組和睡眠剝奪組、正常睡眠組和睡眠恢復(fù)組間差異顯著(P0.05),而睡眠剝奪組和睡眠恢復(fù)組差異不顯著(P0.05);③心境狀態(tài)評(píng)分(7個(gè)分量):睡眠剝奪組和睡眠恢復(fù)組的緊張、憤怒、疲勞差異顯著(P0.05),但是,正常睡眠組和睡眠恢復(fù)組上述差異不顯著(P0.05)。正常睡眠組和睡眠剝奪組疲勞、慌亂和精力差異顯著(P0.05);2.悲傷情緒刺激任務(wù)下激活增強(qiáng)的腦區(qū):(1)睡眠剝奪組-正常睡眠組:左側(cè)扣帶回中部及其側(cè)面環(huán)繞的腦回、小腦腳1區(qū)和前葉4、5區(qū)、旁中央小葉、輔助運(yùn)動(dòng)區(qū)和中央后回;右側(cè)杏仁核、扣帶回前部及其側(cè)面環(huán)繞的腦回,小腦蚓部6區(qū),海馬,額中回、中央前回和內(nèi)側(cè)額上回(中性);右側(cè)內(nèi)側(cè)額上回、扣帶回(前、中部)及其側(cè)面環(huán)繞的腦回,左側(cè)下葉核外白質(zhì)、杏仁核(悲傷);正常睡眠組-睡眠剝奪組:右側(cè)顳葉角回(悲傷);左右兩側(cè)矩狀裂及其周圍皮層;。(2)睡眠恢復(fù)組-正常睡眠組:左側(cè)下葉屏狀核灰質(zhì)、小腦蚓部3區(qū)和海馬;右側(cè)下葉核外白質(zhì)、額葉深部白質(zhì)和小腦前葉4、5區(qū)(悲傷)。左側(cè)顳上回白質(zhì)、小腦腳1區(qū)和小腦后葉8區(qū);右側(cè)矩狀裂及其周圍皮層,中央前回、額葉深部白質(zhì)、輔助運(yùn)動(dòng)區(qū),枕上回,下葉海馬,梭狀回(中性);正常睡眠組-睡眠恢復(fù)組:無(悲傷和中性)。(3)睡眠恢復(fù)組-睡眠剝奪組:左側(cè)額葉羅蘭多殼蓋,右側(cè)顳下回、顳中回(中性);左側(cè)額葉羅蘭多殼蓋區(qū),右側(cè)下葉核外白質(zhì)(悲傷)。睡眠剝奪組-睡眠恢復(fù)組:左側(cè)前扣帶回白質(zhì)、下葉核外白質(zhì)和扣帶回中部及其側(cè)面環(huán)繞的腦回,右側(cè)扣帶回前部及其側(cè)面環(huán)繞的腦回(中性)和無(悲傷)。 結(jié)論: 1.24小時(shí)睡眠剝奪能夠主觀體驗(yàn)到焦慮,緊張、憤怒、疲勞、慌亂和精力下降,而4小時(shí)的睡眠恢復(fù)能緩解其中的緊張、憤怒、疲勞幾乎達(dá)到睡眠正常的水平; 2.女性左右兩側(cè)大腦均參與了對(duì)悲傷情緒的加工,且不同的睡眠狀態(tài)下對(duì)悲傷情緒加工腦區(qū)存在部位、數(shù)量及強(qiáng)度的差異。女性大腦機(jī)能低于正常狀態(tài)時(shí),識(shí)別面孔情緒的能力減低,識(shí)別加工需要更多腦區(qū)域參與,以及更強(qiáng)烈的激活(中性更甚于悲傷,出現(xiàn)悲傷化漂移),這些腦區(qū)包括邊緣系統(tǒng)、額葉、顳葉、小腦。女性大腦機(jī)能處于高于常人水平狀態(tài)時(shí),完成同樣的任務(wù)只需要較少的腦區(qū)參與,表現(xiàn)為節(jié)約及高效; 3.女性悲傷情緒加工,短暫的睡眠恢復(fù)可以選擇地降低睡眠剝奪下腦區(qū)出現(xiàn)的激活強(qiáng)度,,但其部位并非完全重疊,傾向于基本恢復(fù)正常的情緒加工的能力; 4.女性在睡眠剝奪與睡眠恢復(fù)腦區(qū)激活程度與主觀體驗(yàn)之間存在一定差異。
[Abstract]:Objective: To explore the different brain regions in the follicular phase of young women in different sleep states, and to provide a theoretical basis for the correlation mechanism between sleep and mood disorders in women. PARTICIPANTS: A total of 68 healthy female volunteers from Dalian Medical College were recruited as research subjects. According to Pittsburgh Sleep Scale, Minnesota Multipersonality Questionnaire, Special Anxiety Status Questionnaire, Life Events, menstrual cycle curve assessment, 16 healthy female students were screened for entry.
