脊髓受累型多發(fā)性硬化患者靜息態(tài)功能磁共振的局部一致性研究

【摘要】:目的應用靜息態(tài)功能磁共振(RS-f MRI)局部一致性(Re Ho)技術(shù),探討僅有脊髓受累型多發(fā)性硬化患者(MS-SSCI)血氧水平依賴信號的Re Ho。方法對23例MS-SSCI患者(MS-SSCI組)及23名年齡、性別相匹配的健康志愿者(對照組)進行RS-f MRI掃描,比較兩組間Re Ho改變的腦區(qū),進一步分析MS-SSCI患者Re Ho值與臨床擴展殘疾狀態(tài)量表(EDSS)評分及病程間的相關(guān)性。結(jié)果與對照組相比,MS-SSCI患者左側(cè)小腦腳Ⅱ、雙側(cè)顳中回、左側(cè)紡錘體、左側(cè)中央前回、右側(cè)輔助運動區(qū)的Re Ho明顯降低;左側(cè)小腦Ⅸ區(qū)、雙側(cè)尾狀核、左側(cè)后扣帶回的Re Ho明顯升高(雙樣本t檢驗,經(jīng)Alpha Sim校正,P0.01,體素大小40)。相關(guān)性分析結(jié)果顯示,MS-SSCI患者左側(cè)后扣帶回Re Ho值和EDSS呈正相關(guān)(r=0.586,P=0.003)。結(jié)論 MS-SSCI患者靜息狀態(tài)下存在局部腦功能異常,且功能異常與臨床殘疾狀況密切相關(guān),RS-f MRI在探測MS-SSCI腦內(nèi)潛在性病變方面具有重要價值。
