【摘要】：目的探討超聲引導(dǎo)下經(jīng)皮微波消融(MWA)治療甲狀腺良性結(jié)節(jié)的有效性和安全性。方法回顧性分析308例接受超聲引導(dǎo)下經(jīng)皮MWA治療的甲狀腺良性結(jié)節(jié)患者治療及隨訪資料,觀察術(shù)后并發(fā)癥情況,記錄并比較消融前結(jié)節(jié)體積及消融后結(jié)節(jié)體積及體積縮小率。結(jié)果對308例患者共324個結(jié)節(jié)均成功完成消融。術(shù)中及術(shù)后并發(fā)癥患者經(jīng)對癥處理后均恢復(fù)正常。甲狀腺結(jié)節(jié)消融前平均體積為(12.17±20.16)cm~3,最終消融后平均體積為(2.35±4.42)cm~3,較消融前差異有統(tǒng)計學(xué)意義(P0.01),最終消融后體積縮小率為(66.43±60.08)%。消融后1個月、3個月、6個月、12個月甲狀腺結(jié)節(jié)體積均較消融前明顯減小(P均0.05)。消融前結(jié)節(jié)體積與最終消融后結(jié)節(jié)體積縮小率呈正相關(guān)(r=0.181,P=0.001)。結(jié)論超聲引導(dǎo)下經(jīng)皮MWA治療良性甲狀腺結(jié)節(jié)療效確切,且安全、微創(chuàng)。
[Abstract]:Objective to evaluate the efficacy and safety of ultrasound guided percutaneous microwave ablation (MWA) in the treatment of benign thyroid nodules. Methods the clinical data of 308 patients with benign thyroid nodules treated by ultrasound guided percutaneous MWA were retrospectively analyzed. The postoperative complications were observed and the volume and reduction rate of the nodules before and after ablation were recorded and compared. Results all 324 nodules of 308 patients were ablated successfully. Patients with intraoperative and postoperative complications returned to normal after symptomatic treatment. The mean volume of thyroid nodules before ablation was (12.17 鹵20.16) cm~3, and the mean volume after ablation was (2.35 鹵4.42) cm~3,. The volume reduction rate after ablation was (66.43 鹵60.08)%. The volume of thyroid nodule at 1 month, 3 months, 6 months and 12 months after ablation was significantly smaller than that before ablation (P 0.05). There was a positive correlation between the nodule volume before ablation and the reduction rate of nodular volume after final ablation (r = 0.181, P < 0.001). Conclusion Ultrasound-guided percutaneous MWA is effective, safe and minimally invasive in the treatment of benign thyroid nodules.
