黏液基質(zhì)對(duì)肌骨腫瘤表觀擴(kuò)散系數(shù)測(cè)定及診斷價(jià)值的影響

【摘要】:目的探討肌骨腫瘤中黏液基質(zhì)對(duì)其表觀擴(kuò)散系數(shù)(ADC)測(cè)定的影響及ADC值鑒別診斷價(jià)值的影響。方法對(duì)經(jīng)病理證實(shí)的21例富含黏液基質(zhì)的肌骨腫瘤患者術(shù)前行磁共振擴(kuò)散加權(quán)成像(DWI)檢查,測(cè)量腫瘤實(shí)質(zhì)區(qū)ADC值。對(duì)照組為29例其他類不含黏液基質(zhì)肌骨腫瘤。統(tǒng)計(jì)分析富含黏液組與對(duì)照組間ADC值差異,各組中良、惡性腫瘤間的ADC值差異。結(jié)果 (1)富含黏液類腫瘤ADC值均2.0×10-3mm2/s,平均ADC值為(2.32±0.15)×10-3mm2/s,對(duì)照組腫瘤平均ADC值為(1.39±0.37)×10-3mm2/s,兩者間差異有統(tǒng)計(jì)學(xué)意義(t=4.60,P0.05);(2)富含黏液類腫瘤組中,良、惡性腫瘤平均ADC值分別為(2.10±0.21)×10-3mm2/s、(2.05±0.14)×10-3mm2/s,兩者間差異無(wú)統(tǒng)計(jì)學(xué)意義(t=0.68,P0.05)。(3)不含黏液類腫瘤組中,良、惡性腫瘤平均ADC值分別為(1.41±0.36)×10-3mm2/s、(0.94±0.31)×10-3mm2/s,兩者間差異有統(tǒng)計(jì)學(xué)意義(t=1.72,P0.05)。結(jié)論肌骨腫瘤中的黏液基質(zhì)成分可使測(cè)得的腫瘤ADC值增高且2.0×10-3mm2/s。在不含黏液基質(zhì)成分的肌骨腫瘤中,ADC值可用來(lái)對(duì)良、惡性腫瘤鑒別診斷。
[Abstract]:Objective to investigate the effect of mucous matrix on the determination of apparent diffusion coefficient (ADC) and the value of ADC in differential diagnosis of myoskeletal tumors. Methods 21 cases of myoskeletal tumors with mucinous matrix were examined by diffusion weighted imaging (DWI) before operation, and the ADC values of tumor parenchyma were measured. The control group consisted of 29 patients with other types of myoskeletal tumors without mucus matrix. The difference of ADC value between mucus rich group and control group was statistically analyzed. The ADC value of benign and malignant tumor was different in each group. Results (1) the ADC values of mucinous rich tumors were 2.0 脳 10 ~ (-3) mm ~ (-2) / s, the average ADC value was (2.32 鹵0.15) 脳 10 ~ (-3) mm ~ 2 / s, and the average ADC value of control group was (1.39 鹵0.37) 脳 10 ~ (-3) mm ~ 2 / s. The mean ADC values of malignant tumors were (2.10 鹵0.21) 脳 10 ~ (-3) mm ~ 2 / s, (2.05 鹵0.14) 脳 10 ~ (-3) mm ~ (2 / s) respectively. There was no significant difference between the two groups (t _ (0.68) P _ (0.05). (_ 3). The mean ADC values of benign and malignant tumors were (1.41 鹵0.36) 脳 10 ~ (-3) mm ~ (2 / s) and (0.94 鹵0.31) 脳 10 ~ (-3) mm ~ (2 / s) respectively. Conclusion the mucus matrix in myosseous tumor can increase the ADC value of the tumor and 2.0 脳 10 ~ (-3) mm ~ (2 / s) 路s ~ (-2) 路s ~ (-1). ADC value can be used in differential diagnosis of benign and malignant tumors without mucus matrix.
