【摘要】：目的探討磁共振動態(tài)增強成像(DCE-MRI)與乳腺磁共振報告數據系統(BI-RADS-MR)在乳腺腫塊診斷中的價值。方法對經病理證實的75例乳腺腫塊患者共計75個腫塊術前行DCE-MRI檢查,并測定動態(tài)增強參數。由兩名高年資影像診斷醫(yī)師采用盲法閱片,并根據BI-RADS-MR進行分級,構建受試者工作特征曲線(ROC)分析兩名醫(yī)師的判讀情況,評價兩名醫(yī)生診斷乳腺腫塊的靈敏度、特異度。結果根據BI-RADS-MR分級,38例乳腺癌患者評為Ⅰ級1例,Ⅱ級0例,Ⅲ級4例,Ⅳ級13例,Ⅴ級20例;良性Ⅰ級17例,Ⅱ級5例,Ⅲ級12例,Ⅳ級3例,Ⅴ級0例。良惡性乳腺腫塊的T_(peak)、S_(max)、K_1差異有統計學意義(均P0.05)。兩名醫(yī)師根據BI-RADS-MR評分方法及DCE-MRI診斷乳腺癌的ROC曲線下面積(AUC)為0.940,95%置信區(qū)間為0.887~0.994。BI-RADS-MR評分方法診斷乳腺癌的靈敏度為94.59%、特異度為92.11%。結論 DCE-MRI參數與BI-RADS-MR評分方法對于提高乳腺腫塊診斷及鑒別診斷效能具有重要價值。
[Abstract]:Objective to evaluate the value of dynamic enhanced magnetic resonance imaging (DCE-MRI) and magnetic resonance reporting data system (BI-RADS-MR) in the diagnosis of breast masses. Methods 75 cases of breast masses confirmed by pathology were examined by DCE-MRI before operation and dynamic enhancement parameters were measured. Two senior imaging diagnostics were selected to read the films by blind method. According to the BI-RADS-MR classification, the operating characteristic curve was constructed to analyze the interpretation of the two doctors, and the sensitivity and specificity of the two doctors in the diagnosis of breast masses were evaluated.
