【摘要】：目的探討磁共振動(dòng)態(tài)增強(qiáng)成像(DCE-MRI)與乳腺磁共振報(bào)告數(shù)據(jù)系統(tǒng)(BI-RADS-MR)在乳腺腫塊診斷中的價(jià)值。方法對(duì)經(jīng)病理證實(shí)的75例乳腺腫塊患者共計(jì)75個(gè)腫塊術(shù)前行DCE-MRI檢查,并測(cè)定動(dòng)態(tài)增強(qiáng)參數(shù)。由兩名高年資影像診斷醫(yī)師采用盲法閱片,并根據(jù)BI-RADS-MR進(jìn)行分級(jí),構(gòu)建受試者工作特征曲線(ROC)分析兩名醫(yī)師的判讀情況,評(píng)價(jià)兩名醫(yī)生診斷乳腺腫塊的靈敏度、特異度。結(jié)果根據(jù)BI-RADS-MR分級(jí),38例乳腺癌患者評(píng)為Ⅰ級(jí)1例,Ⅱ級(jí)0例,Ⅲ級(jí)4例,Ⅳ級(jí)13例,Ⅴ級(jí)20例;良性Ⅰ級(jí)17例,Ⅱ級(jí)5例,Ⅲ級(jí)12例,Ⅳ級(jí)3例,Ⅴ級(jí)0例。良惡性乳腺腫塊的T_(peak)、S_(max)、K_1差異有統(tǒng)計(jì)學(xué)意義(均P0.05)。兩名醫(yī)師根據(jù)BI-RADS-MR評(píng)分方法及DCE-MRI診斷乳腺癌的ROC曲線下面積(AUC)為0.940,95%置信區(qū)間為0.887~0.994。BI-RADS-MR評(píng)分方法診斷乳腺癌的靈敏度為94.59%、特異度為92.11%。結(jié)論 DCE-MRI參數(shù)與BI-RADS-MR評(píng)分方法對(duì)于提高乳腺腫塊診斷及鑒別診斷效能具有重要價(jià)值。
[Abstract]:Objective to evaluate the value of dynamic enhanced magnetic resonance imaging (DCE-MRI) and magnetic resonance reporting data system (BI-RADS-MR) in the diagnosis of breast masses. Methods 75 cases of breast masses confirmed by pathology were examined by DCE-MRI before operation and dynamic enhancement parameters were measured. Two senior imaging diagnostics were selected to read the films by blind method. According to the BI-RADS-MR classification, the operating characteristic curve was constructed to analyze the interpretation of the two doctors, and the sensitivity and specificity of the two doctors in the diagnosis of breast masses were evaluated.
